Job Summary
The Clinical Liaison serves as the primary clinical and marketing representative for Cornerstone Post Acute Care of Bossier, responsible for developing and maintaining referral relationships with hospitals, physicians, case managers, discharge planners, payors, and other healthcare providers. The Clinical Liaison evaluates potential patient referrals, conducts clinical assessments to determine appropriateness for post-acute services, and facilitates a seamless admission process for patients requiring Long-Term Acute Care (LTAC), Skilled Nursing Facility (SNF), and Rehabilitation services.
The Clinical Liaison acts as a trusted resource to referral sources, patients, and families by providing education regarding available services, coordinating care transitions, and ensuring timely communication throughout the referral and admission process. The Clinical Liaison collaborates closely with clinical staff, admissions personnel, case management teams, and leadership to promote quality patient outcomes and enhance the organization's reputation as a leader in post-acute healthcare services
Education and/or Experience:
· Experience as a RN/LVN/RT in critical care, med/surg setting, preferred
· Sales Team Member within a Sales and Marketing arena, preferred

- Knowledge of emergency and disaster procedures of the facility
- Maintain accountability for activities, including statistics and regular activity reports on a weekly basis
- Maintain documentation that is in compliance with all regulatory accrediting agencies
- Participate in hospital wide patient safety program identifying risks to patient safety and reducing healthcare errors
- Perform other duties as assigned
