Home Care Manager and Care Navigator
The Home Care Manager is a key role that builds rapport with older adults and caregivers throughout our service area. The Home Care Manager provides leadership to home care staff and provides care consultation and navigation to participants at our community center located in Silver Bay, MN.
North Shore Area Partners is seeking a self-motivated, reliable, and highly organized team member who possesses a passion for mission driven work. The Home Care Manager is a key role that builds rapport with older adults and caregivers throughout our service area. The Home Care Manager will provide care consultation and support via phone, at our office, or by meeting older adults and caregivers in their homes, which includes assessing overall wellbeing, risk factors and safety concerns, and educating older adults and caregivers on resources, identify support networks, and assist with paperwork in order to help maintain safe, independent living. The Home Care Manager will also provide primary leadership and oversight of our Home Care Program ensuring regulatory compliance and quality of care. The ideal candidate is capable of managing their own workload, prioritizing tasks, and thinking ahead to meet the needs of a growing organization.
Description of Primary Responsibilities:
1. Home Care Management and Oversight
· Maintain thorough knowledge of 245D licensure requirements and scope of care.
· Assess the home care needs of clients, identifying and developing specific plans of care.
· Perform initial and ongoing in-home care evaluations and supervision to ensure that quality service is provided to meet client needs.
· Oversee the supervision, scheduling, training, and performance of direct care staff.
· Communicate with clients, family members, direct care staff and other professionals involved in the care of the client.
· Maintain, monitor, and verify accurate home care records, including service documentation, attendance, medical records, and billing; ensure confidentiality and HIPAA compliance. Responsible for submitting EVV statistics to Department of Human Services as directed.
· Understand and adhere to established North Shore Area Partners policies and procedures; develop or adapt new policies and procedures as necessary for licensing requirements.
· Ensure suspected maltreatment reports are filed and reported as legally mandated.
2. Care Consultation and Caregiver Counseling
· Maintain thorough knowledge of information and referral of community-based services for older persons, their family members and other caregivers, including a thorough knowledge of the services and programs provided by North Shore Area Partners.
· Provide consultation to older adults and caregivers via phone or in-person visits; includes completing an intake and assessment, assistance in setting goals, identifying needs, determining potential sources of support (informal and formal), making appropriate referrals, and making a care plan assistance to enable them to live independently in their own home.
· Assist with paperwork and practical needs relating to housing needs, financial assistance, health insurance, and health care directives.
· Provide Individual and Family Counseling which is assistance provided to caregivers in making decisions and solving problems related to their caregiver roles.
· Provide Caregiver Coaching/Consulting which is an individualized support service that equips caregivers with the knowledge, skills, and tools to perform their caregiving role while achieving a balanced lifestyle.
· Provide active listening and support to address grief, loss, and mental health concerns.
· Provide follow-up visits as often as needed to achieve their goals
· Provide Telephone Reassurance as needed.
· Facilitate communication between participants, volunteers, and other staff.
3. Program Support and Development
· Attend regular staff meetings and lead weekly Care Team meetings.
· Maintain participant records and provide data for all participant contact and service provision.
· Complete monthly NAPIS reports and Quarterly Narrative Report per III-B & III-E requirements and other applicable grant requirements.
· Provide information to Executive Director on participant activities as necessary for quarterly and annual reports to funders and Board.
· Write articles for newsletters and other mediums as needed providing program updates.
· Provide project/event coordination and/or leadership support by helping internal and external parties organize various components needed to initiate, run, and conclude projects.
· Assess program service systems, make recommendations, and offer solutions to revise or modify systems to better meet organizational and participant needs.
· Maintain positive work atmosphere by behaving and communicating in a manner that fosters good relations with participants, volunteers, co-workers, supervisory staff, and community members.
· Ensure that all activities are conducted in compliance with all applicable Federal and State laws, rules and regulations, HIPPA, including Civil Rights.
4. Community Outreach and Public Relations
· Positively represent the agency and promote North Shore Area Partners’ services in the community as requested.
· Attend meetings and special trainings as identified by Executive Director.
· Network with other providers of aging services to stay current on available resources.
5. Perform other Duties as assigned.
Education and Experience:
· Bachelor’s degree in Social Work, Psychology, or health/human service related field, and two years supervising/scheduling required.
· Experience working with older adults and caregivers.
· Dementia and Caregiver Consulting training or willingness to be trained.
· Proficiency in Microsoft Office applications (Outlook, Word, Excel, Teams, etc.).

Preferred Knowledge, Skills and Abilities:
· Patience and compassion for older adults in need of assistance.
· Knowledge of resources in Lake County.
· Excellent written and oral communication skills.
· Excellent organizational and clerical skills.
· Knowledgeable in 245D licensure and compliance.
Physical Requirements
· Ability to communicate both orally and in writing.
· Work at a desk for extended periods of time.
· Able to travel to locations/homes in North Shore Area Partners’ service area on a regular basis.
· Move around the office to support staff, perform various tasks, and room set-up.
· Requires sitting, standing, bending, stooping, and reaching with sufficient manual dexterity to operate standard office machines such as computers, fax machines, calculators, copy machines, telephone and other office equipment.
Other Details:
· 32 hours/week (.8 FTE)
· $28-$32/hour (based on education and experience).
· Paid holiday, vacation and sick leave, accrued based on hours worked.
