Now Hiring: Full-Time Home Health Intake Coordinator

Silver City, NM (Local Candidates Only)

Schedule: Monday–Friday | 8:00 AM – 5:00 PM MST

Join Addus Home Health and become part of a team dedicated to delivering compassionate, high-quality care. As an Intake Coordinator, you’ll play a vital role in supporting patients and families by coordinating services that help individuals recover, manage chronic conditions, and remain safely in the comfort of their own homes.

Key Responsibilities

Receive and process patient referrals by phone and in person
Explain home health services and benefits to patients, families, and referral sources
Assist in identifying alternative services when a patient is not eligible for care
Coordinate and schedule initial nursing assessments and admissions
Prepare and maintain admission packets and required documentation
Support community outreach efforts and maintain strong referral relationships
Provide administrative and clerical support to the clinical team
Maintain accurate patient records and documentation in a timely manner

Qualifications

Must be at least 18 years of age
One (1) year of experience in home health, healthcare, or a related field preferred
Strong organizational, communication, and customer service skills
Ability to manage multiple priorities in a fast-paced environment
Ability to complete documentation accurately and on time
Valid driver’s license and proof of auto insurance required
