This position will covering the greater Bangor area. Required Wednesdays in office in Pittsfield.

We’re looking for a compassionate, motivated, and organized Community Integration Specialist to join our team!

As a Community Integration Specialist, you’ll work directly with clients to identify their needs, develop individualized goals, coordinate services, monitor progress, and provide support that empowers individuals to build on their strengths and become as independent as possible.

This is a rewarding opportunity for someone who enjoys working independently while being part of a supportive team and who brings professionalism, compassion, and strong communication skills to their work.

What You'll Do

As a Community Integration Specialist, you will:

Assess client needs and complete comprehensive assessments.
Develop individualized care plans, including crisis plans.
Complete ANSA assessments every 90 days.
Connect clients with appropriate community resources and services.
Coordinate care and collaborate with providers involved in client care.
Regularly communicate with service providers to ensure client needs are being met.
Support clients and families in identifying and building upon their strengths, resources, and natural supports.
Provide services using trauma-informed, person-centered, and strengths-based practices.
Respect each client's privacy, individuality, choices, culture, and personal goals.
Monitor client safety, well-being, stability, and progress toward established goals.
Maintain accurate and timely client records in accordance with agency policies, applicable laws, licensing requirements, MaineCare regulations, and client rights.
Independently manage an assigned caseload, including scheduling appointments, meetings, and time for required documentation.
Maintain professional boundaries and ethical relationships with clients.
Participate actively in supervision, training, and professional development.
Maintain a minimum of 25 billable hours per week.

What We're Looking ForRequired Qualifications

MHRT/C or MHRT/CP certification required.
High school diploma/GED or higher.
Reliable transportation.
Strong written and verbal communication skills.
Ability to work independently and manage a caseload effectively.
Ability to collaborate successfully with clients, families, providers, and community partners.
Working knowledge of Microsoft Office, Outlook, Zoom, and other video conferencing platforms.

Preferred Qualifications

Two or more years of case management experience.
Experience working with individuals receiving mental health or behavioral health services.
Knowledge of MaineCare requirements and community-based behavioral health services.
