Summary

This non-exempt supervisory position, in cooperation with the Connections Advocate Coordinator is responsible for ensuring Direct Support Professionals (DSPs) are provided support which promotes dignity and respect; is directed with strengths and needs in mind; and creates opportunities for growth. The Connections Advocate will be trained with the clients they supervise and may cover shifts or work events anytime within a 24/7 spectrum. The position facilitates community inclusion through group/individual social activities and select case management for people that receive waiver services in a community setting. This includes identifying desired activities, transport to events/activities, providing social coaching specific to the individual’s social goals, and documenting billable services. This position is also responsible for ensuring that policies, procedures, and practices comply with applicable state and federal law and are aligned with Frontier’s commitment to person-directed services.
Responsibilities

Supervise DSPs assigned to caseload, including coordinating interviews and conducting performance appraisals
Provide weekly reflective supervision in the initial 90-day probationary period; quarterly thereafter
Provide ongoing supervision, coaching, and mentoring to DSPs to support quality service delivery; develop performance improvement plans as needed
Conduct disciplinary processes in collaboration with HR and Senior Management
Ensure DSPs have required orientation and training (individual-specific, Global Hazard Systems, Safety Data Sheets)
Lead quarterly meetings for supervised staff (community and group home DSPs)
Participate in hiring events with HR and management
Participate in committee activities, community events, and agency communication
Problem-solve for shift coverage with Connections and Learning/Scheduling Department
Provide coverage for client shifts (evenings, weekends, holidays)
Respond to DSPs after hours, weekends, and holidays on on-call rotation
Send schedule updates to Schedule Change
Create and implement ongoing DSP schedules
Schedule and complete 10+ hours of ongoing service hours with clients
Provide case management assistance reflecting individuals’ choices and preferences (e.g., access to medical, education, vocational services, important documents)
Schedule, plan, and provide services as outlined in care plans and bill for services
Check recruitment needs regularly; if needs are not up to date, forward information to the appropriate person
Conduct home visits to assess case management and service needs
Assist clients in identifying, accessing, and coordinating services outside FCS Waiver program, monitor services provided, and follow up with providers
Complete annual assessments per state, grant, and COA requirements
Conduct meet-and-greets for new hires related to caseload
Ensure documentation of noteworthy events and activities in accordance with agency policy
Convene and facilitate person-centered planning meetings
Develop and update service goals and objectives in AlayaCare
Monitor health and safety of clients; complete and submit reports (Critical Incident Reports, APS/CPS reports, etc.)
Facilitate socialization within community and statewide through waiver day habilitation services
Implement scheduled activities, provide transportation, and supervise social, educational, and recreational activities
Encourage individuals to develop skills and participate in activities that enhance community living, interpersonal skills, and social relationships
Monitor clients’ physical and emotional well-being; report concerns to care team
Participate in treatment team development of treatment goals, behavior support, and crisis/safety plans
Write day habilitation notes and complete required documentation (service notes, medication logs, client files)
Assist with personal care needs and medication administration as required
Maintain confidentiality of client information in accordance with agency policy
Notify social media coordinator weekly of activities for agency posting
Participate in evening, weekend, holiday, and multi-day activities as needed
Collaborate with care teams to ensure seamless provision of services
Maintain and reconcile receipts for activities and transportation
