Full Job Description

About Us:
Founded in 2010, Hahn Home Health Care Inc. provides high-quality, private pay home care services across Southeastern Massachusetts. Our mission is to deliver peace of mind to families by offering compassionate care for clients of all ages and needs. We are now seeking a motivated and dynamic Community Liaison to help us expand our outreach and foster relationships with key referral sources.

Position Overview:
As a Community Liaison, you will be responsible for driving business development through outreach, relationship-building, and referral generation for both private pay non-medical home care and private pay skilled nursing services. You’ll work directly with decision-makers at rehab centers, nursing homes, senior living facilities, elder care attorneys, financial planners, and other community organizations to introduce and promote our services.

This is a part-time, field-based position (Monday-Friday, 9:00 AM to 3:00 PM) with some flexibility and occasional weekend or evening work. Your time will be split between in-person visits (about 15 per week) and administrative tasks like documenting visits and developing marketing strategies.

Key Responsibilities:

· Referral Generation: Build and nurture relationships with potential referral sources, including rehab centers, senior communities, elder care professionals, and financial planners. Your goal is to ensure a steady stream of referrals for private pay home care and private pay skilled nursing services.

· Market Outreach: Conduct marketing visits to a minimum of 15 locations per week. Maintain detailed records of interactions, including notes on the referral potential and next steps.

· Marketing Strategy: Develop and execute a localized marketing strategy, identifying key referral sources and crafting targeted outreach plans to build awareness of our services.

· Community Engagement: Attend and represent Hahn Home Health Care at relevant community events, networking functions, and industry meetings (1-2 per month).

· Digital Presence: Manage and grow our online presence, including social media and email communications, to increase awareness and engagement.

· Reporting & Planning: Regularly update your visit logs and provide weekly reports on activities, progress, and upcoming goals. Collaborate with the team to adjust strategies as needed.

Qualifications:

· Experience: At least 1 year of experience in marketing, business development, nursing, or social work, ideally within healthcare or home care.

· Knowledge: Familiarity with the private pay home care industry is preferred.

· Skills: Strong interpersonal skills, with the ability to build relationships with diverse community stakeholders.

· Other Requirements:

o Valid driver’s license, car, and current auto insurance (reliable transportation is required).

o Must be able to travel locally within the assigned region.

Compensation & Benefits:

· Hourly Pay: $25 - $27 per hour, commensurate with experience.

· Bonus Structure: Performance-based bonuses based on referral generation.
