Full Job Description

We seek an experienced Lead Justice-Involved CHW Coordinator to support and lead community-based health services to justice-involved individuals, focusing on reentry support, health education, and linkage to care. The successful candidate will oversee CHWs, facilitate program development, and deliver effective service to justice-involved clients.

Key Responsibilities:

1. Program Oversight:

a. Lead Justice-Involved CHWs, providing guidance and support.

b. Assist with developing and implementing program policies, procedures, and standards.

c. Assist with the interview and onboarding process for new CHWs.

d. Ensure compliance with funding requirements and regulations.

2. Client Services:

a. Coordinate healthcare services for justice-involved clients.

b. Facilitate linkages to community resources, healthcare providers, and social services.

c. Ensure client-centered approach and cultural competency.

3. Staff Development:

a. Provide training, coaching, and mentorship to CHWs.

b. Foster teamwork, collaboration, and professional growth.

c. Conduct performance evaluations and address concerns.

4. Community Partnerships:

a. Establish and maintain partnerships with justice system stakeholders, healthcare providers, and community organizations.

b. Facilitate referrals, collaborations, and resource sharing.

5. Quality Improvement:

a. Monitor program outcomes, identifying areas for improvement.

b. Implement quality enhancement initiatives and evaluate effectiveness.

6. Reporting and Evaluation:

a. Track program metrics, including client outcomes and CHW performance.

b. Prepare reports for funders, stakeholders, and internal teams.

Other Responsibilities:

1. Develop and maintain relationships with JI clients, providing support and guidance throughout their reentry process.

2. Conduct client assessments to identify needs and develop personalized plans.

3. Connect clients with community resources, including healthcare, housing, employment, and social services.

4. Provide education and support on health and wellness topics, such as substance abuse, mental health, and chronic disease management.

5. Facilitate group sessions and workshops on topics relevant to JI clients.

6. Collaborate with healthcare providers, social services, and justice system partners to ensure comprehensive support.

7. Maintain accurate records and reports, including client progress and outcomes.

8. Participate in ongoing training and professional development to enhance skills and knowledge.

Essential Skill set required:

1. Cultural Competence: Understanding the JI population's unique needs, values, and beliefs.

2. Trauma-Informed Care: Knowledge of trauma's impact and ability to provide sensitive support.

3. Communication Skills: Effective communication with JI clients, healthcare providers, and social services.

4. Assessment and Referral: Ability to assess client needs and connect them with appropriate resources.

5. Reentry Planning: Understanding of reentry challenges and ability to develop personalized plans.

6. Health Education: Knowledge of health and wellness topics, including substance abuse and mental health.

7. Navigation and Advocacy: Ability to navigate healthcare and social service systems, advocating for clients' needs.

8. Documentation and Record-Keeping: Accurate documentation of client interactions and progress.

9. Boundary Setting: Ability to maintain professional boundaries while building trust with clients.

10. Flexibility and Adaptability: Willingness to adapt to changing client needs and circumstances.

11. Collaboration and Partnership: Ability to work with diverse stakeholders, including healthcare providers, social services, and justice system agencies.

12. Crisis Intervention: Knowledge of crisis intervention techniques and ability to respond to client emergencies.

13. Motivational Interviewing: Understanding of motivational interviewing principles to support client engagement.

14. Health Literacy: Ability to communicate complex health information in an accessible manner.

15. Technology Skills: Familiarity with electronic health records and other digital tools.

Qualifications:

1. Education: Bachelor’s degree in social work, Public Health, Psychology, or a related field.

2. Experience working with JI populations or similar vulnerable groups.

3. Community-based work or in-vivo work is required with proper safety precautions.

4. Certification as a Community Health Worker (CHW) or willingness to obtain within 18 months.

5. CPI and CPR within 60 days of hire.

6. Stable vehicle and insurance.

Required Experience:

1. A strong understanding of the justice system, community resources, and health disparities.

2. Strong knowledge of Orange County's healthcare, social services, and correctional facilities landscape.

3. Ability to work effectively with diverse populations and stakeholders.

4. Familiarity with community resources and services.

5. Proficient in electronic health records and data management

Preferred Qualifications:

1. Master’s degree in social work, Public Health, human service, criminal justice, or a related field.

2. Lived experience in the justice system or similar challenges.

3. Bilingual or multilingual skills.

4. Experience with motivational interviewing, trauma-informed care, or other evidence-based practices.

5. Knowledge of health and wellness topics, such as substance abuse, mental health, and chronic disease management.

Working Conditions:

1. Remote and Office environment for administration work.

2. CHW services are provided virtually or in person with locations in any setting, including correctional facilities, reentry programs, courts, foster homes, residential facilities, group homes, outpatient clinics, hospitals, homes, or community settings.

3. Monday through Friday (8 hours), after hours, and weekends to accommodate clients' schedules, including special events and outreach.

4. Travel is necessary to other counties and community locations, if applicable.

5. A small caseload will be maintained between 15-20 clients, and this may be higher to temporarily assist with overflow and proper CHW connection.
