Duties and Responsibilities

The One You Love Homecare Bridgewater Registered Nurse (RN) is responsible for the clinical oversight of the Agency’s personal care services program. The RN oversees and manages cases and services rendered by caregivers.

The RN interacts with clients and serves as a liaison between clients and staff. The RN manages initial nursing assessments, re-assessments, case monitoring visits and caregiver supervisory visits. All clients are assessed and Plan of Cares managed per regulatory requirements.

Responsibilities:

1. Assess new clients, develop Plan of Care, direct level of care, and assign Plan of Care tasks.

2. Evaluate records relating to the care provided to the client on a regular basis. Conduct service calls with the client and/or significant other at least every thirty days to ensure the Plan of Care continues to meet client needs.

3. Conduct an in-person reassessment of the client every 6 months and case monitoring visits every 60 days, or more frequently if the client’s condition warrants it or there has been an interruption of service due to a hospitalization or stay in a medical facility.

4. Conduct and manage competency testing, develop and manage in-service training programs, and complete performance evaluations for caregivers.

5. Maintain documentation as per the regulatory requirements.

6. Maintain good working relationships with clients, family members, and caregivers, focusing on retention (retaining both clients and caregivers).

7. Coordinate in-services with the Administrator/Owner for additional training for caregivers.

8. Be available for on-call duty, providing a nursing consultation by phone and doing an in-home visit as needed.

9. Maintain active state licensure, completing all required Continued Education Units, per state regulations.

10. Be the Nursing Supervisor of the agency and be listed on the agency license with the state of New Jersey.

General Position Requirements

Current valid license in good standing.
Experience in providing nursing services in a patient’s home environment, inclusive of an assisted-living facility.
Good phone skills.
Ability to coordinate and organize multiple tasks.
Good communication skills.
Self-motivation.
Ability to work closely with the Director/Owner and Agency Manager.
Familiarity with computers and specific software programs related to the job.

Pay: $35.00 - $45.00 per hour
