Full Job Description

Job Title: Medical Biller & Coder

Position Summary

The Medical Biller & Coder is responsible for reviewing patient encounters, assigning accurate medical codes, entering charges, and ensuring timely and compliant claims submission. This role supports revenue cycle operations by maintaining data accuracy, correcting claim errors, and monitoring claim acceptance through clearinghouse and payer systems, in alignment with established billing and coding workflows.

Essential Job Functions

1. Patient Encounter Review & Eligibility Verification

Review medical records to verify completeness of documentation for each patient encounter
Confirm patient insurance eligibility and active policy status
Validate prior authorizations and ensure authorization numbers match scheduled procedures

2. Medical Coding

Assign accurate ICD-10-CM diagnosis codes with highest level of specificity
Assign appropriate CPT and HCPCS procedure codes and link to diagnosis codes
Apply correct medical coding modifiers as applicable
Perform CCI (Correct Coding Initiative) edits checks to ensure code compliance and prevent bundling issues

3. Charge Entry

Verify patient demographic information (name, DOB, insurance/self-pay status)
Enter rendering, referring, and billing provider information, including NPIs
Input validated ICD-10, CPT, and modifier codes into billing system
Link procedure codes to appropriate diagnosis codes
Verify accuracy of date of service (DOS) across all documentation and claims

4. Claim Scrubbing & Pre-Submission Audits

Review daily patient schedule and reconcile total charges entered
Identify and correct front-end billing errors prior to claim generation
Balance daily batches and ensure alignment between coded encounters and entered charges

5. Electronic Claims Submission

Prepare claims for submission by setting status to “ready to submit”
Transmit claims electronically to clearinghouse via billing software
Review transmission reports/logs to confirm successful submission of claim batches

6. Clearinghouse Review & Claim Follow-Up

Review clearinghouse reports (e.g., 277 reports) to monitor claim status
Identify and correct rejected claims and resubmit promptly
Track claims through payer adjudication to ensure acceptance
Submit Bi-Weekly and Monthly reports to our Controller with claim updates and Accounts Receivable.

Qualifications

High school diploma or equivalent required; Associate degree in Health Information Management or related field preferred
Minimum 2+ years of medical billing and coding experience in a healthcare setting preferred
Certification preferred (e.g., CPC, CCS, or equivalent)
Strong knowledge of ICD-10-CM, CPT, HCPCS, and payer guidelines
Experience with electronic medical records (EMR) and billing systems specially EcW

Knowledge, Skills, & Abilities

Strong attention to detail and accuracy in data entry and coding
Ability to interpret medical documentation and apply appropriate coding standards
Knowledge of insurance verification and authorization processes
Familiarity with claims submission workflows and clearinghouse operations
Ability to identify and resolve billing errors efficiently
Strong organizational and time management skills

Physical & Work Requirements (ADA Compliant)

Ability to sit for extended periods while working at a computer
Frequent use of hands for typing and data entry
Ability to view computer screens for prolonged periods
Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions

Work Environment

Office-based or healthcare administrative setting
Minimal physical exertion required
Frequent interaction with billing systems, EMRs, and administrative staff

Compliance & Standards

Adhere to HIPAA regulations and patient confidentiality requirements
Ensure compliance with all federal, state, and payer-specific billing and coding guidelines
Follow internal SOPs and revenue cycle procedures for accuracy and audit readiness
