Full Job Description

Case Manager RN

Position Summary
The Case Manager RN coordinates multidisciplinary care for members with complex medical and social needs. Serving as the primary point of contact for members, caregivers, and providers, this role develops, implements, and monitors individualized care plans to improve health outcomes across the continuum of care. Care is delivered primarily through phone and digital communication.

Key Responsibilities

Assess members' medical, behavioral, and psychosocial needs.
Develop, implement, and monitor individualized care plans.
Coordinate care with providers, caregivers, and community resources.
Educate members on disease management, medications, nutrition, and self-care.
Identify and address barriers to care and connect members with appropriate services.
Support care transitions, discharge planning, and continuity of care.
Advocate for members while promoting self-management and improved health outcomes.
Document all care management activities accurately and meet quality and productivity standards.
Collaborate with an interdisciplinary team including social workers, dietitians, pharmacists, and medical directors.

Qualifications

Active, unrestricted Michigan Registered Nurse (RN) license required.
Associate Degree or Nursing Diploma required; BSN preferred.
Minimum 3 years of clinical nursing experience in acute, post-acute, community, or clinical settings.
Case management experience in managed care preferred.
Experience providing telephonic and digital care management preferred.
CCM certification required within 18 months of hire (preferred upon hire).
Strong communication, critical thinking, organizational, and computer skills with the ability to manage multiple priorities in a fast-paced environment.

Pay: $37.00 - $39.00 per hour

Associate (Required)

Experience:

Registered Nurse: 3 years (Required)
Case Management: 1 year (Preferred)
Clinical Nursing: 3 years (Required)

Work Location: Remote
