Full Job Description

We are seeking a compassionate and organized Patient Navigator to join our team at Holisticare. As a Patient Navigator, you will play a critical role in assessing patient needs, developing care plans, and coordinating services and resources for patients. You will work closely with doctors and other healthcare professionals to ensure high-quality patient care.

Responsibilities:

Primary care coordinator for Chronic Care Management (CCM) program.
Monitor patients’ chronic conditions.
Connect patients with the services they need and follow up on necessary services.
Make sure the patient receives timely recommended preventative services.
Oversee the patients’ medication management.
Communicate with patients' families and caretakers as needed.
Answer patient calls, emails, and questions.
Assist with setting goals for the patients.
Create a care plan with a patient’s clinical & non-clinical needs.
Confidentially manage patient accounts.
Ensure compliance with professional standards & regulatory requirements.
Work as a team player to ensure each patient receives the best care possible.
Provide community resources and referrals for continuing care.
Participate in committee meetings and projects.
Coordinates care with agency personnel and updates team as required.
Futuristically, oversee other CCM Care managers as the team expands.

Requirements:

Medical/ Clinical background required
Strong communication and organization skills
Problem-solving and resourcefulness capabilities
Stellar follow-up skills and critical thinker
Previous training or experience in coordination or healthcare a plus
