Full Job Description

CSO is looking for a Case Manager to join our PACT team in Northampton. Located in the heart of downtown with access to shops, busses and restaurants.

Program for Assertive Community Treatment (PACT) is an evidence-based intensive outpatient treatment program designed to assist adults overcome the barriers to their recovery from severe and persistent behavioral health disorders. This position will provide intensive case management for adults who have DMH's highest level of community supports. The goal of PACT Services is to support clients in the community by reducing the number of inpatient hospitalization.

The ideal candidate will have familiarity with completing various applications including housing, employment, SNAP, SSI and SSDI.

This is a full-time, Monday-Friday position. 8:30am-4:30pm. No on-call, no weekends, no holidays!

Starting salary: $38,000-$44,000/year

As a PACT Case Manager:

You Will:

Provide motivational counseling, prompting, and intensive support to consumers during psychiatric crises or periods of transition; monitor and assist clients with grocery shopping and meal preparation, and household activities
Provide education and support with identifying community resources including appropriate mental health, addiction and medical services, and accessing services of choice
Provide skills training and assistance in areas of daily living activities including basic living and interactive skills, providing "in vivo" skills training, side by side assistance, vocational and pre-vocational support to clients to include identifying interests and strengths and goals, and development of individualized placements. Assist in developing communication and conflict resolution
Provide assistance with developing and maintaining a "natural support network" of family and friends
Provide intervention treatment services as part of a multidisciplinary team
Assist, train and support clients in locating affordable housing, scheduling healthcare appointments
May provide recovery support services including exploring treatment recovery options with clients
Provide transportation and necessary support for client appointments
Maintain appropriate contacts with clients and their natural supports

Qualifications

Bachelor's degree in a behavioral health discipline or related field
Valid driver's license and reliable transportation
