Are you a dedicated and caring individual with a passion for making a positive impact on the lives of older adults and/or people with disabilities? If so, we have the perfect opportunity for you to join our growing and compassionate case management team in the Windsor, VT area!

At Senior Solutions (Area Agency on Aging in Southeastern Vermont), we are seeking a Case Manager to work with older adults and/or people with disabilities in the Windsor and the surrounding areas using a person-centered planning approach and a deep understanding of state and federal benefits. Strong organizational, communication, and computer skills are required.

Qualifications:

Bachelor’s degree in human services and prior case management experience preferred.

A combination of relevant education and experience may be substituted for a degree.

We are always looking for team members who are driven to quickly learn new skills about all aspects of the benefits and services available to aging Vermonters. Must have reliable transportation and a valid driver's license.

Responsibilities include:

Collaborating with other community partners to create a person-centered plan of care.
Monitoring and reassessing program service plans as required.
Having a working knowledge of services and programs available to older Vermonters and people with disabilities.
Assisting, when necessary, to gain access to existing services such as public benefits, housing, legal services, etc.
Ensuring that all provided services do not exceed budgeted amounts and are consistent with DAIL guidelines.
