Position Summary
As the Licensed Practical Nurse at Health Hub Clinic, you are the clinical right hand to our physicians and a trusted care partner to every member who walks through our doors. This role is the operational and clinical heartbeat of our Direct Primary Care (DPC) practice — combining hands-on nursing skill with the relational continuity that defines DPC.
You won't be pushed through a high-volume revolving door. You'll actually know our members, anticipate their needs, and play a central role in delivering the kind of unhurried, thorough care that makes DPC different — while keeping our clinical workflows sharp, safe, and efficient.
Key Responsibilities
Direct Patient Care
- Rooming & Vitals: Room patients efficiently, obtain accurate vital signs, complete chief complaint documentation, and prepare patients for provider visits.
- Clinical Procedures: Perform venipuncture, injections, immunizations, point-of-care testing (strep, flu, COVID, urinalysis, glucose, A1c), wound care, EKGs, ear lavage, and other in-office procedures within LPN scope.
- Medication Administration: Administer medications, injections, and vaccines per provider orders and Missouri LPN scope of practice.
- Patient Education: Reinforce provider instructions, review medications, and coach members on chronic condition self-management, preventive screenings, and lifestyle interventions.
Care Coordination & Continuity
- Chronic Care Management: Support members with diabetes, hypertension, thyroid, and other chronic conditions through regular check-ins, labs, and follow-up outreach.
- Referral & Specialist Coordination: Facilitate outbound referrals, secure prior authorizations, and track referral loop closure so members don't fall through the cracks.
- Lab & Imaging Follow-Up: Track outstanding labs and imaging orders (including studies at Health Guardian Imaging), flag abnormal results for the provider, and communicate results to members clearly.
- Message & Refill Triage: Triage patient messages, phone calls, and refill requests using standing orders and provider guidance.
Clinical Operations & Compliance
- EMR Documentation: Maintain accurate, timely, HIPAA-compliant charting in Electronic Medical Record.
- Supply & Inventory: Manage clinical supply inventory, stock exam rooms, monitor medication expiration dates, and oversee vaccine storage and temperature logs.
- Infection Control & Safety: Enforce OSHA and infection-control standards; maintain equipment cleanliness and sterilization logs.
- Emergency Preparedness: Support clinic emergency preparedness protocols and respond appropriately to in-office urgent clinical situations.
Team Contribution & DPC Culture
- Provider Partnership: Work as an extension of the physician(s) to keep visits on time, thorough, and focused on the member.
- Member Relationships: Build genuine, long-term trusting relationships with members.
- Continuous Improvement: Identify workflow gaps, suggest clinical protocol refinements, and contribute to a culture of Operational Excellence.
Required Qualifications
- Current Missouri LPN license in good standing
- Graduation from an accredited practical nursing program
- Current BLS/CPR certification
- 1+ year of clinical nursing experience (primary care, family medicine, or outpatient preferred)
- Proficient with Electronic Medical Record systems and general computer literacy
- Strong written and verbal communication skills
- Reliable attendance and ability to work in-person during operating hours
Preferred Qualifications
- Direct Primary Care, family medicine, internal medicine, or concierge practice experience
- Experience with chronic care management and preventive health
- Familiarity with point-of-care testing, phlebotomy, injections, and immunization protocols
- Working knowledge of medical terminology, coding basics, and HIPAA compliance
- Experience with prior authorizations, referrals, and specialist coordination
- Bilingual (English/Spanish) a plus

Physical Requirements
- Ability to sit, stand, and walk for extended periods
- Occasional lifting up to 50 lbs. (assisting patients, moving supplies)
- Manual dexterity for clinical procedures, injections, and EMR use
- Ability to assist with patient transfers and mobility support as needed
