Job Overview
The Kind Place is seeking a dependable and compassionate Case Manager / Community Resource Specialist to provide home and community-based support for adults with brain injuries.
Our work is focused on helping people build independence while being treated with dignity, patience, and respect. This position is a good fit for someone who is professional, emotionally mature, resourceful, comfortable working independently, and able to communicate respectfully with adults from many different backgrounds. Knowledge of community resources such as SSI/SSDI, Medicaid, Medicare, food stamps, housing, shelters, food pantries, transportation, and other supports is helpful.
This role involves providing practical support, helping clients problem-solve, and assisting with daily tasks, appointments, paperwork, community resources, and skills that support greater independence.
Responsibilities
Responsibilities may include:
- Provide direct support to adults in home and community settings.
- Help clients improve independence, organization, memory, follow-through, and daily functioning.
- Support clients with calendars, reminders, written schedules, paperwork organization, phone alarms, and other accommodations.
- Assist with practical life skills such as budgeting, grocery shopping, meal planning, household tasks, scheduling appointments, and completing follow-up tasks.
- Help clients understand and navigate community resources, benefit programs, housing resources, food assistance, medical systems, transportation options, and other local supports.
- Support clients in scheduling and attending appointments with physicians, social service agencies, attorneys, community agencies, and other providers as appropriate.
- Transport clients in your own vehicle as needed for appointments, errands, and community activities.
- Communicate professionally with clients, families, providers, and community partners.
- Monitor and report on client progress, needs, concerns, and overall well-being.
- Complete accurate and timely documentation of services provided.
- Maintain appropriate boundaries and use good judgment in community-based work.
Requirements
- High school diploma or equivalent required.
- Bachelor’s degree preferred.
- Experience in case management, social services, disability services, brain injury support, mental health, healthcare, or a related field preferred.
- Knowledge of community resources preferred.
- Must be dependable, professional, emotionally mature, and able to work independently.
- Must have strong communication and interpersonal skills.
- Must be compassionate, respectful, and comfortable working with adults from many different backgrounds.
- Must be resourceful and willing to learn about community programs and support systems.
- Must have reliable transportation and be able to transport clients in your own vehicle.
- Flexibility to work some evenings or weekends as needed is helpful.
