We are seeking a highly motivated, innovative, enthusiastic, and inclusive staff member to join our team as a Chronic Disease Program Coordinator.
The Bilingual Chronic Disease Program Coordinator position will be responsible for building relationships with communities across Larimer County, implementing programming to reduce the burden of chronic disease, and aligning these efforts with the CDPHE Chronic Disease grant.

What you'll be doing:

Coordinate and administer program services that address chronic disease through collaboration with community organizations and directly with community members.

Deliver culturally-responsive support and communication in English and Spanish.

Build and and maintain diverse partnerships, establishing collaborative goals, and resolving conflicts.

Collaborate with the Strategic Planning, Health Equity, and Partnerships team to create innovative and responsive programming across the county.

Collect data, evaluate program effectiveness, assess the needs of the community, and recommend improvements to program procedures and services to meet those needs.

Commit to learning about and engaging in conversations about racial and health equity and disparities and how they relate to social determinants of health.

Conduct marketing efforts in partnership with the Office of Communications and Technology, to promote public awareness and education on program services including developing or refining webpages, developing and distributing educational materials, developing outreach plans, and making presentations to community groups and collateral agencies.

Develop and present educational/informational materials related to chronic disease including, fact sheets, newsletters, advertisements, web information, and talking point papers.

Three (3) years experience in a directly related field or in the performance of similar duties.

Possess reliable transportation to be able to work in any County facility as needed and required.

Ability to communicate clearly and concisely in both English and Spanish, both verbally and in writing, with correct usage of spelling, punctuation, grammar, and cultural awareness.

Schedule
The standard work schedule for this position is Monday through Friday, 8:00 AM to 4:30 PM. During the initial six-month probationary period, the individual hired may telework up to one day per week, if business needs allow. After successfully completing the probationary period, the individual may telework up to two days per week, depending on business plans and operational needs.
Additionally, this position includes assignments that require in-community work, which may affect the hybrid remote schedule and involve hours worked outside of standard business hours. Your specific schedule will be discussed with you to ensure it aligns with the needs of the role and the business.
This position is grant-funded. The grant is renewed on a three-year cycle beginning in 2026. We anticipate continued funding beyond the current grant period; however, future renewals are not guaranteed.
