Full Job Description

Duties

Provide direct supervision and support to SH clinical team
Ensure implementation of evidence-based interventions, behavioral supports, and therapeutic services.
Oversees Stabilization Programs, weekly meeting notes, Transition Plans, Closing Summaries, and other required reports, meeting assigned timeframes.
Review clinical stabilization documentation for quality, accuracy, and compliance.
Monitor clinical outcomes and identify opportunities for service enhancement.
Completes assessments for individuals on assigned caseload, meeting assigned timeframes.
Facilitate weekly interdisciplinary stabilization meetings involving guardians, ISCs, DHS representatives, providers, clinical teams, and support staff.
Review stabilization progress and barriers to discharge.
Assist in developing individualized stabilization plans and transition strategies.
Ensure compliance with DHS, CILA, and IAG standards.
Monitor incident trends, restrictive interventions, hospitalization data, and crisis utilization.
Participate in licensing reviews, audits, and quality assurance activities.
Support implementation of corrective action plans, when needed.
Mentor and develop Clinical Directors, QIDP Implementers, Behavior Analysts, and other clinical support staff.
Provide coaching related to trauma-informed care, motivational interviewing, positive behavior supports, and crisis intervention.
Assist with onboarding, training, and professional development initiatives.
Support relationship building across service systems to ensure continuity of care.
Assists in training new Direct Support Professionals (DSPs).
Provides ongoing training to DSPs as needed in areas such as the SKILLS System, abuse and neglect, or other clinically relevant matters.
Coordinates and facilitates transition trainings with providing agency, family (as needed), ISC, and SST.
Attends psychiatric appointments, ensuring proper documentation is submitted to the provider and, in conjunction with the RN, following up on all recommendations.
Attends SH meetings regarding individuals as scheduled.
Responds to crises as required or assigned.
Leads in such a way as to maximum the independence of individuals and to maintain their rights according to IAG policy and the Department of Human Services regulations.
Communicates with guardians, Independent Service Coordinators (ISCs), Support Services Team (SST), behavior analysts, nurses, and supervisory personnel.
Completes required documentation accurately and on time.

· Obtains annual CEs as required by DHS.

· Attends appointments or accompanies people to the emergency room or urgent care as assigned.

· Achieves competency or certification in first aid, CPR, Medication Administration, and other required skills within prescribed timeframes.

· Substitutes for absent co-workers in the performance of assigned duties with direction from supervisor.

· Performs miscellaneous duties as assigned by supervisor in order to ensure day-to-day functioning of the home.

· Submits required employee records on time.

· Attends all required trainings and meetings.

Minimum Requirements

Education and Experience:

Minimum of master’s degree in counseling, Social Work, Psychology, Behavioral Health, Rehabilitation Counseling, or related Human Services field
Illinois clinical licensure (LCPC, LCSW, or Psychologist) preferred.
Minimum of three years’ directly related work experience

Licenses and Certifications:

· Must possess a valid driver’s license.

· Must possess current automobile insurance.

· Must be over 25 to operate an agency vehicle.

Skills and Knowledge:

· Possess strong interpersonal skills for interactions with supervisors, coworkers, individuals, and outside personnel.

· Communicate effectively (verbally and in writing) with supervisor, coworkers, individuals, and outside personnel.

· Follow procedures for delivery of instructional, training, and support services.

· Demonstrate knowledge and appropriate implementation of each designated individual’s needs, goals, interventions and supports.

· Complete required record-keeping with accuracy.

· Efficient use of time, materials, and human resources.

Pay: $80,000.00 per year
