This role is pivotal in the organization and execution of client-centered discharge and transition planning
for clients receiving stabilization services. The Crisis Care Navigator is responsible for prioritizing identified
client needs and ensuring that all relevant team members are coordinating and documenting
interventions. On-unit and care coordination interventions are provided with the express purpose of
assisting clients with long-term recovery efforts which extend beyond the client’s need for crisis
stabilization services. This position is central to ensure that the necessary resources and documentation
is complete and delivered in a timely manner.
JOB DUTIES AND RESPONSIBILITIES:
This position is responsible and held accountable for the following duties:
- Maintain and monitor standardized communication processes on the unit.
- Act as the primary liaison between the crisis unit and internal and external care coordination
treatment partners.
- Provide oversight to unit staff to ensure completeness of documentation required for seamless
transition for clients between levels of care as well to external referring partners.
- Maintain working knowledge of local referral sources related to treatment and social services to meet
clients’ basic needs.
- Ensure completeness of Release of Information and related chart documentation related to identified
parties (e.g., contact logs, Care Coordination Care Pathway, Contact Information, etc.).
- Complete or ensure completion of assessments to help identify and prioritize client’s resources
needs for stabilization.
- Identifies charting gaps and communicates and delegates gaps appropriately to meet client needs in a
timely manner.
- Communicate with outpatient providers when high utilization clients admit to the crisis stabilization
unit.
- Collaborate with crisis unit staff and utilizations specialists to assist in dispositioning client insurance
needs, including but not limited to; identifying insurance, entering the Payer Care Pathway, assisting in
applying for insurance, etc.
- Utilizes Eleos, the agency’s AI supported scribing tool, for eligible services.
- Understands and promotes our Certified Community Behavioral Health Clinic (CCBHC) concept by
providing or supporting community-based mental and substance use disorder services; advancing
integration of behavioral health with physical health care; assimilating and utilizing evidence-based
practices on a more consistent basis; and promoting improved access to high quality care.
- Performs other reasonably related duties as assigned by the immediate supervisor or other
management as requested.
- Meet or exceed “satisfactory” expectations at any scheduled job performance evaluation.
- Regular and predictable attendance is essential and expected for this position.
Performance Metrics
- 95% of transitions to the Addiction Recovery Center charts have all required charting completed.
- 90% of clients dispositioned have a release of information or referral entered for a Primary Care
Provider.
>90% of clients dispositioned have a follow up appointment scheduled with outpatient service
providers.
- >95% of clients have an appropriate CDC entered at time of transition which accurately reflects the
outcome of their transition.
Must perform the specific job duties as listed above to meet position expectations.
WORK ENVIRONMENT:
- This position works mainly in an indoor environment. This position will be working in an office setting.
- This role will require occasional travel (up to 25%) to various community locations and in the client’s
residence as needed for the role (e.g., schools, government offices, medical facilities, etc.)
- Must be willing and able to perform all job-related travel normally associated with this position (this
may include transportation of clients).
- Travel expectations can vary from time to time based on coverage needs
EDUCATION, QUALIFICATIONS, AND EXPERIENCE:
- High School Diploma or equivalent required
- Required to be certified through Oklahoma Department of Mental Health and Substance Abuse
Services (ODMHSAS) as a Certified Peer Recovery Support Specialist (CPRSS), Certified Family Peer
Recovery Support Specialist (CFPRSS) or Case Management (CM) I; CM II preferred.
- Current and valid Driver’s License
- Maintain driver’s license and have a clean driving record and reliable transportation
KNOWLEDGE, SKILLS, AND ABILITIES:
- Proficiency in healthcare documentation and charting standards.
- Working knowledge of confidentiality standards related to HIPPA, CFR 42 Pt 2, Oklahoma Title 43A, as
well as all relevant statutes and regulations.
- Ability to successfully navigate an Electronic Health Record and identify gaps in documentation.
- Ability to effectively facilitate collaboration between a multidisciplinary treatment team.
- Ability to maintain high documentation standards in a crisis unit environment.
- High communication and delegation skills.
- Ability to work in a fast-paced treatment environment.
- Ability to communicate effectively, both orally and in writing.
- Ability to communicate using a trauma informed, person-centered approach with clients, staff, and
community partners.
- High degree of conflict resolution and alliance building to help create collaboration and cohesion in a
team-based environment.
- Proficient use of Eleos and other agency approved documentation technology as a required function of
this position. In addition, must possess skill in working cooperatively and effectively with clients, staff, management,
and other professional groups; in exercising mature judgment in dealing with people; in presenting ideas
clearly and accurately; in reading and comprehending the English language;
