Overview

We are seeking a dynamic and compassionate Wellness Director Must be a LPN to lead our comprehensive wellness programs within a long-term care environment. This vital role involves overseeing resident health initiatives, managing clinical teams, and ensuring the delivery of exceptional care that promotes residents’ physical, emotional, and social well-being. The Wellness Director will serve as a key leader in fostering a vibrant, supportive community where residents thrive and feel valued. This position offers an exciting opportunity for a motivated healthcare professional to make a meaningful impact on residents’ lives while demonstrating strong leadership and operational expertise.

Duties

Develop, implement, and oversee wellness programs tailored to residents’ needs, including physical activity, mental health support, and social engagement initiatives.
Supervise nursing staff and care teams, providing guidance on resident care plans, documentation review, and compliance with long-term care regulations.
Ensure adherence to HIPAA regulations and maintain accurate medical records using EMR/EHR systems.
Coordinate with interdisciplinary teams to manage resident’s <PERSON>;, ensuring personalized care that aligns with medical terminology and physiology knowledge.
Monitor regulatory compliance related to Medicare, Medicaid, and state long-term care standards; conduct regular audits of medical records and documentation.
Lead staff training on nursing administration, shift management, and regulatory updates to foster a knowledgeable and compliant team.
Manage care facility operations related to memory care, hospice care, home health services, and acute care requirements.
Review care plans regularly to ensure quality patient care; oversee the management of medical records and documentation review processes.
Promote a culture of continuous improvement by analyzing clinical outcomes and implementing best practices for patient care.

Experience

Proven experience in assisted living or long-term care facilities with demonstrated leadership in nursing management or healthcare administration.
Extensive background in nursing home experience, home health agencies, hospice care, or acute care environments.
Supervising experience managing multidisciplinary teams within a healthcare setting; strong nurse management skills are essential.
Familiarity with EMR/EHR systems, medical terminology, anatomy knowledge, and physiology is required for effective documentation review and patient management.
Knowledge of long-term care regulations, Medicare policies, HIPAA compliance standards, and regulatory reporting procedures.
Prior experience managing care facility teams with an emphasis on regulatory compliance and quality assurance.
Strong understanding of patient-centered care approaches including memory care strategies; experience with managing complex cases involving multiple comorbidities is preferred.
Demonstrated leadership in nursing administration with the ability to motivate staff while maintaining high standards of clinical excellence.
Excellent communication skills for collaborating with residents’ families, healthcare providers, and regulatory agencies.
