Full Job Description

Job Summary: The Targeted Case Manager provides comprehensive care coordination and support services to individuals and families, ensuring access to needed resources and services. This role focuses on assessing client needs, developing individualized service plans, and coordinating care across internal programs and external providers to promote stability, independence, and overall well-being. TCM works collaboratively with internal staff, community partners, and service providers to ensure clients receive comprehensive, client-centered care. TCM promotes the organization’s mission while advocating for client well-being, stability, and independence. This role is also responsible for ensuring services are properly documented, compliant with funding and regulatory requirements, and eligible for billing in accordance with Medicaid and other payer guidelines.

Responsibilities:

1. Conduct comprehensive assessments to identify client strengths, needs, and goals

2. Develop, implement, and regularly update individualized service/treatment plans

3. Coordinate and monitor services across internal departments and community providers

4. Serve as the primary point of contact to ensure continuity of care from intake through discharge

5. Advocate on behalf of clients to access appropriate services and supports

6. Maintain regular contact with clients through home visits, office visits, and/or virtual check-ins

7. Monitor client progress and adjust service plans as needed

8. Maintain accurate, timely, and compliant documentation in accordance with agency and regulatory standards

9. Collaborate with multidisciplinary teams to ensure integrated care

10. Provide crisis intervention and support as necessary

11. Ensure compliance with all applicable federal, state, and local regulations, including Medicaid guidelines

12. Attend regular staff meetings, clinical staffing, mandatory trainings as assigned, and other meetings as required.

13. Participation in agency Quality Improvement program

14. Attend weekly supervision—with all case notes and required information current and completed in full.

15. Must complete Wraparound Certification within 6 months of hire date.

Physical Responsibilities:

Ability to sit and stand

The ability to drive to other locations as needed

Equipment Used:

1. Standard office technology and equipment, including laptop/computer, printer, scanner, and multifunction devices.

2. Common productivity software and digital platforms as required for daily tasks.

3. Personal cell phone.

Qualifications:

1. Bachelor’s degree in social work, Human Services, Psychology, or related field required

2. 2-3 years of experience in case management, care coordination, or human services

3. Knowledge of Florida Medicaid Targeted Case Management billing requirements and documentation standards preferred.

4. Ability to meet productivity and billable hour expectations.

Ability to manage a caseload and prioritize tasks effectively
Proficiency in electronic health records (EHR) or case management systems
Valid driver’s license and reliable transportation
Adhere to established safety procedures and practices in the work area.
