We achieve this by creating care goals with clients and their families, and meeting those goals through supportive programs and practices, providing essential daily services, and working to improve the well-being and quality of life for those we serve. Our team finds a strong sense of purpose in their work and fosters deep connections with their coworkers, clients, and communities.

Touching Hearts at Home won the Top Workplace award from the Milwaukee Journal Sentinel for 2018, 2019, 2020,2021, and earned 1st place for 2022, 2023, 2024, 2025 and 2026! We provide in home non-medical care to seniors and adults with disabilities. We serve the Milwaukee, Waukesha, and Ozaukee county areas. Our office is located in New Berlin however most work will be out in the field or from home. We started the business in 2007 and have been locally owned and operated ever since.

Full job description

For the Care Coordinator position, we are seeking a compassionate, organized, and relationship-driven Care Coordinator who is deeply committed to communication, collaboration, and creative problem-solving. This role is critical to the growth and sustainability of our care model, which is built on one core belief: trusting relationships are essential with clients, caregivers, and our internal team.

From managing new client inquiries to maintaining oversight of care documentation and caregiver communication, this role ensures a consistent, high-quality experience for all involved. The ideal candidate is someone who thrives in conversation, communicates firmly yet compassionately, and has the confidence and heart to address challenges head-on — always seeking creative solutions through a positive and collaborative lens.

Whether you're conducting a client consultation, navigating a delicate family dynamic, or coaching a caregiver through a tough moment, your words and actions will set the tone for what compassionate care looks like.

Key Responsibilities

Team Communication & Coordination

*Be the communication hub for the care team — ensuring messages are clearly conveyed, appropriately documented, and followed through.
*Proactively engage with colleagues, caregivers, and leadership with strong verbal and written communication.
*Address concerns and feedback with firmness, compassion, and curiosity — always open to different perspectives.
*Encourage and model a collaborative problem-solving approach, especially in complex or emotionally charged situations.

Client Inquiries & Lead Management

*Serve as the first point of contact for prospective clients —listening with empathy and responding with intent.
*Respond promptly and professionally, offering same-day or next-day consults when possible.
*Ensure communication flows effectively between intake, operations, and care teams.
*Build trusted relationships with referral sources, reflecting our values through every interaction.

Client Consults & On-boarding

*Conduct in-home consults to create care plans that will help clients achieve goals of remaining in their homes, living independently as long as possible.
*Guide families through care options and pricing with transparent, firm, and compassionate communication, navigating sensitive conversations with empathy, clarity, and confidence.
*Collaborate with leadership to problem-solve around complex challenges, transitions, and evolving client needs.

Caregiver & Client Oversight

*Conduct regular client visits and check-ins to evaluate care quality and nurture relationships.
*Maintain clear, consistent communication with caregivers to ensure accurate care delivery and documentation.
*Track and document all changes in care, caregiver availability, hospitalizations, or incidents.
*Address issues directly and collaboratively, viewing them as opportunities for growth, learning, and care improvement.
*Act as a steady presence and trusted communicator for both clients and caregivers.
*Provide reassurance and clarity during emotionally difficult moments (e.g., end-of-life care, service transitions).

Community Recruitment

This will only be about 15% of your role. This will entail attending some job fairs, church events, community events, and some work in the local job portals online. The goal is to help bring in some wonderful local caregivers who are not necessarily looking for work on Indeed for example. This could be fun and an opportunity to give the role some additional diversity!

Minimum Qualifications:

* Bachelor's degree (Preferred)

* 1 year of experience in Client Care Management/Coordination (Required)

* Auto Insurance (Required)

* Driver's License (Required)

* Ability to pass a Background Check

* Requires regular travel throughout Milwaukee, Waukesha, and Ozaukee Counties.

*Primarily a 1st-shift position, but flexibility is needed to provide support responding to issues that arise outside typical office hours

Preferred Qualifications:

* Experience working in a community/social service agency.

* Knowledge of or experience with the geriatric demographic.

* Variety of experiences working effectively with others from different backgrounds and cultures.

* Basic computer skills demonstrating comfortability with word processing, database, spreadsheet, and supportive software.

* Excellent oral and written comprehension and communication skills.

* Detail orientated with exceptional time-management skills

* Skilled in building trust, fostering teamwork, and providing ongoing support.

* Strong interpersonal skills to engage, collaborate, and maintain strong relationships with both coworkers and clients and their families.

* Human relations skills sufficient to deal effectively with complex situations; and to understand, develop, motivate, collaborate, and lead others with integrity.

*Adept in problem solving with a positive and collaborative approach.
