Patient Care Coordinator
POSITION SUMMARY

We are searching for a Patient Care Coordinator (PCC) – someone who works well in a fast-paced setting. In this position, you will be responsible for providing and coordinating all tasks associated with patient services, including but not limited to patient intake, scheduling, navigation, and follow-through completion of the recommended Plan of Care at an Outpatient Physical Therapy Practice. The incumbent serves as the link between patients and the clinic. You will ensure optimal patient outcomes that address quality, service, customer satisfaction and cost effectiveness.

POSITION KEY ACCOUNTABILITY

1. Responsible for answering high volume phone calls during the course of business and greeting patients.

2. Assists with the completion of the patient scheduling process to ensure optimal efficiency in clinic operations and resources while maintaining the Practice’s standards of customer service and quality patient care.

3. Effectively optimize the schedule, securing authorizations (pre-certifications), and verifying clinic specific requirements are available. Effectively communicate pending authorizations and place patient’s appointments on hold. Update the clinical and admin team accordingly.

4. Reporting: Prepares and monitors outcome data to assist in identification of improvement opportunities.

5. Attention to Detail: Track referrals and referral sources in our CRM web application (Salesforce). Enters patient’s demographics and insurance details in our EMR computer systems (WebPT and Revflow), with a high degree of accuracy.

6. Accounting: Protects the financial integrity of the facility by collecting patient liability (co-pays, deductibles and co-insurances), establishing payment arrangements (requires management approval), and discussing other payment options.

7. Communication: Uses positive and effective interpersonal skills when dealing with patients/families, visitors, peers, and external healthcare team members.

8. May assist patients with requests for transportation assistance and resolve other barriers to care.

9. Responsible for the security and confidentiality of patient information at all times.

10. Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and supports practice-based goals which contribute to the overall success of the company.

11. Other duties as assigned.

CERTIFICATION/SKILLS

1. Knowledge of appointment scheduling, commercial and private based healthcare, auto PIP policies, Workers Comp programs, and has the ability to read/interpret patient’s insurance benefits and eligibility.

2. Logical and mathematical aptitude. Accurately complete daily reconciliation of collections.

3. Ability to work in multiple computer software programs simultaneously; including but not limited to MS Word, MS Excel and Adobe.

4. Ability to provide translation services in Spanish a plus.

5. Strong customer service skills with the ability to communicate effectively both verbally and in writing. Proficient typing/keyboarding skills.

6. Must be able to work independently and exercise sound judgment, in a high stress environment. Must meet deadlines and perform all daily assignments with consistent accuracy.

7. Ability to establish and maintain effective working relationship with patients, clients, clinical/non-clinical staff, attorneys and insurance companies.

8. Excellent telephone etiquette is required.

9. Strong prioritization and organizational skills; extreme attention to detail.

10. Requires dependability, flexibility and teamwork.

MINIMUM EDUCATION

High school diploma or equivalent.

MINIMUM EXPERIENCE

Two years of related experience, which includes healthcare and administrative support. May substitute required experience with equivalent years of education beyond the minimum education requirement.

PHYSICAL REQUIREMENTS

Must be able to sit at a computer terminal for extended periods of time.

Occasionally lift/carry items weighting up to 20 lbs.

Occasionally assist patients with mobility problems.

SECURITY SENSITIVE

Maintains privacy and confidentiality per HIPAA guidelines.

Pay: $16.00 - $18.00 per hour
