Full Job Description

Sumter East Health & Rehabilitation Center is seeking a dedicated and skilled Wound Care Nurse, Registered Nurse to join our healthcare team. The ideal candidate will possess a strong background in nursing with specialized knowledge in wound care management. This role is critical in providing high-quality care to patients with complex wounds, ensuring optimal healing and comfort. The Wound Care Nurse will work collaboratively with other healthcare professionals to develop and implement individualized care plans. This is a Weekend position only !

Responsibilities

Assess and evaluate patients' wounds, documenting findings accurately in medical records.
Develop, implement, and monitor wound care treatment plans based on best practices and patient needs.
Provide education to patients and their families regarding wound care management and prevention strategies.
Perform advanced wound care techniques, including but not limited to dressing changes, debridement, and gastric lavage when necessary.
Collaborate with interdisciplinary teams, including primary care providers, hospice palliative medicine specialists, and assisted living staff, to ensure comprehensive patient care.
Conduct specimen collection processing as needed for laboratory analysis.
Maintain accurate medical records and documentation of all patient interactions and treatments provided.
Participate in quality improvement initiatives related to wound care practices within the facility.

Requirements

Current Registered Nurse License (RN ) with relevant experience in wound care or a related field.
Strong knowledge of anatomy and physiology as it relates to wound healing processes.
Experience in primary care settings or hospice palliative medicine is preferred.
Familiarity with Minimum Data Set (MDS) assessments is a plus.
Ability to catheterize patients when required for specific treatments.
Excellent communication skills for effective patient education and collaboration with healthcare teams.
Strong organizational skills with attention to detail for accurate documentation and record keeping.
