Full Job Description

JOB TITLE: Licensed Practical Nurse

REPORTS TO: Director of Nursing

DEPARTMENT: Health Center

GENERAL SUMMARY

Provides direct nursing care to residents and delegates and supervises the care provided by RN. Administers medications, performs treatments and procedures, obtains specimens as needed, completes documentation as required. Shift: 10 pm to 6 am.

ESSENTIAL JOB DUTIES

Document resident status as per policy.
Provide daily supervision to assigned staff and directly provide nursing care to residents as assigned.
Participate in the admission of new residents.
Assist residents with Activities of Daily Living (ADL), family/social activities and actively promote independent life-style of residents.
Respond to residents call system promptly and courteously.
Responsible for identification of resident medical and behavioral changes and the development and monitoring of care plans and re-assessment of each resident.
Provide daily report to the Director of Health Services concerning facility conditions, personnel performance, resident conditions, and needs for nursing supplies.
Provide nursing care according to physician’s orders and in conformance with recognized nursing practice, established standards, and administrative policies.
Ensure preventative, therapeutic and rehabilitative services to residents and assists in the coordination of nursing care with care rendered by other disciplines.

ADDITIONAL DUTIES

Keeps physician and family aware of change in resident’s condition.
Maintains the self‑respect, personal dignity, and physical safety of each patient.
Assures continuity of services and care.
Attends in-service training and education sessions, as assigned.
Performs specific work duties and responsibilities assigned by supervisor.

EDUCATION/ QUALIFICATIONS/ EXPERIENCE

LPN currently licensed by the State.
Fingerprints clearance card and CPR Certification required.
Geriatric involvement experience preferable.

COMPETENCIES

Knowledge of EMR software.
Must be able to work paying close attention to detail with frequent interruptions.
Willingness to be available for any/all emergencies regarding the facility.
LANGUAGE SKILLS
The ability to speak, read and comprehend the English language.
MATHEMATICAL SKILLS
Must have ability to compute basic mathematical equations.
ACCESS TO PERSONAL HEALTH INFORMATION
Level 3: Full access to the Medical Record subset of the Designated Record Set.

PHYSICAL REQUIREMENTS/WORKING CONDITIONS:

This job operates in a skilled nursing facility.
Must be able to continuously stoop, bend, stretch, squat, stand, and walk for up to 70% of the work day.
Must be able to lift, transfer, turn and position residents weighing up to 125 lbs. unassisted, or with the assistance of one, for a resident weighing over 125 lbs.
Must be able to lift and carry up to 20 lbs. frequently and 50 lbs. occasionally.
May be subjected to offensive odors and combative behavior.
May be exposed to communicable diseases including the HIV and HBV viruses.
