Full Job Description

Position Title: Case Manager

Supervisor: Program Director

Location: Holiday FL

Work Schedule: Mon-Fri 8am to 5pm

Job Classification: Full-Time Nonexempt

Pay Rate: $21

Starting Date: ASAP

SUMMARY: The Case Manager provides direct services to individuals facing homelessness . This role involves assessing the needs for clients to be able to be back to self-sufficiency by coordinating community resources, developing service plans, and offering emotional support while guiding clients through the process.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Conduct comprehensive assessments of clients to determine their immediate and long-term needs.
Develop individualized service plans that outline specific goals, timelines, and resources for each client.
Coordinate services and resources such as housing, financial assistance, mental health counseling, and medical care. Collaborate with local, state, and federal agencies, as well as community organizations, to meet clients' needs.
Maintain accurate and detailed case files for each client, documenting all interactions, services provided, and progress made. Ensure compliance with agency policies and funder requirements.
Provide emotional support and crisis intervention to individuals helping them navigate their situation and reduce stress.
Act as an advocate for clients to ensure they receive appropriate assistance and support from community resources and relief organizations.
Make referrals to appropriate local agencies, housing programs, employment services, and other needed resources.
Engage in outreach efforts to ensure that underserved and vulnerable populations are aware of available services.
Work closely with other case managers, volunteers, and team members to ensure a coordinated approach.
Monitor clients’ progress and follow up regularly to adjust service plans as needed and ensure successful outcomes.
Participate in ongoing training and professional development related to case management, and trauma-informed care.
Works in a spirit of cooperation with all external and internal stakeholders.
Achieve individual productivity requirements, contribute appropriately to program productivity objectives, and use independent judgment.
Complete and update case records for each client in accordance with agency Guidelines.
Represent the program in appropriate meetings, public forums, partner agency meetings, workshops, etc.
Network with other community providers regarding program services, and referrals of clients.
Responsible for reports as required by the Agency and funder in a timely thorough manner.

OTHER RESPONSIBILITIES

Complies with all applicable training requirements.
Complies with all company safety, personnel and operational policies and procedures.
Complies with work schedule to ensure effective operations of Agency programs.
Contributes positively as a member of a productive and cooperative team.
Participates in Agency Performance Quality Improvement (/PQI) program.
Performs other duties as necessary to fulfill Catholic Charities Diocese of St Petersburg, Inc Mission.

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES:

Able to speak, write and understand English.
Must be sensitive to and respect cultural diversity amongst clients, staff, and volunteers and able to work with diverse racial, ethnic, and economic groups.
Flexible work schedule including evenings, nights, weekends, and holidays.
Ability to set appropriate limits, work under deadlines and multi-task.
Ability to organize, prioritize, self-motivate, and deliver results.
Excellent communication and listening skills.
Possess strong work ethics.
Successfully pass a FDLE Level II background screening.
Valid Florida driver’s license, ability to drive van or bus and have reliable transportation.

A high level of openness and willingness to receive feedback/suggestions from superiors and others, and to learn new skills to improve job performance.

EDUCATION AND EXPERIENCE

Bachelor’s degree in social work, psychology, or a related field. Equivalent experience in disaster relief or case management may be considered.
Strong leadership and networking skills
1-2 years of case management experience, ideally in disaster relief, emergency services, or social services.
Thorough knowledge of community services and target populations.
