Location: Greater Tampa Bay — Hillsborough, Pinellas, and Pasco, Polk, Highlands, Hardee, Manatee, DeSoto, Sarasota, Charlotte, Collier, Glades, Hendry Lee Counties (office + field)

About the Role

A growing provider of in-home and community supports for individuals with intellectual and developmental disabilities is looking for a self-starting Community Partnership & Care Coordination Specialist. This is a hybrid office-and-field role for someone who can keep cases organized and get out into the community.

You will match caregivers to families served through Florida Medicaid waiver and managed-care programs, visit homes to check that care is working, fix problems with families when they come up, recruit caregivers at job fairs and hiring events, and build trusted relationships with support coordinators, case managers, and other referral partners across the Suncoast.

This is not a desk-only job. The right person is comfortable in the office one day and at a family’s kitchen table, a career fair, or a coordinator’s office the next.

What You Will Do

Manage day-to-day case paperwork, authorizations, schedules, and documentation so services stay compliant and families stay supported.
Source, screen, and match caregivers to individuals based on skills, personality, language, schedule, and the plan of care.
Visit clients in the field to assess needs, confirm quality of care, and troubleshoot issues with individuals, families, and field staff.
Represent the organization at job fairs, hiring events, and community gatherings to attract reliable caregiver talent.
Build and maintain professional relationships with Waiver Support Coordinators, case managers, managed-care contacts, and other community partners so families are confidently referred and well served.
Work closely with the Administrator on service quality, staffing coverage, and the organization’s growth priorities.
Keep accurate notes and follow-through so nothing falls through the cracks between office, field, and partners.

Who You Are

Self-motivated, organized, and comfortable owning both paperwork and people.
A hustler and a relationship-builder — you like meeting people in person and you follow up.
Experienced working with individuals with intellectual or developmental disabilities, in-home supports, APD / iBudget, Medicaid waiver, or Florida managed-care programs — especially in the Suncoast region — is a strong plus.
You already know how this community works: coordinators, families, hiring events, and local partners. Existing professional relationships you can put to work from day one are highly valued.
Bilingual English/Spanish is a significant plus, not a requirement.
Valid driver’s license, reliable transportation, and willingness to travel throughout Hillsborough, Pinellas, and Pasco Counties.
Able to pass Florida Level 2 background screening (AHCA Clearinghouse).
