Job Overview

We are seeking compassionate and dedicated Direct Care Workers to provide exceptional support and care to individuals with diverse needs in various settings, including residential facilities, assisted living, and home health environments. This role is vital in promoting independence, safety, and well-being for clients with disabilities, chronic illnesses, or age-related conditions. The ideal candidate will possess a strong commitment to patient-centered care, excellent communication skills, and the ability to work effectively both independently and as part of a team.

Duties

Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting
Monitor patients’ vital signs and observe changes in health status
Support residents with mobility using patient transfer methods including Hoyer lifts and other assistive devices
Prepare nutritious meals in accordance with dietary restrictions and care plans
Maintain a clean and safe environment through cleaning and sanitation tasks
Document patient progress accurately and communicate effectively with healthcare teams
Provide emotional support and companionship to enhance quality of life
Assist with transportation for medical appointments or community outings
Follow safety protocols including first aid, CPR, manual handling, and emergency procedures

Skills

Prior experience working in group homes, nursing homes, assisted living, or home health care preferred
Knowledge of dementia care, Alzheimer’s care, memory care, and neurodevelopmental disorders such as autism preferred
Ability to perform patient observation
Experience with medication administration and care planning
Skilled in assisting with ADLs and personal care assistance
Familiarity with HIPAA regulations to ensure client confidentiality
Competence in patient transfer methods including Hoyer lifts and patient transfer techniques
Strong communication skills for effective client interaction and documentation
Ability to handle heavy lifting safely and perform manual handling tasks as required
Knowledge of geriatrics, developmental disabilities support, hospice care, and long-term care practices
First aid and CPR certification is highly desirable; ongoing training provided if needed
