Job Summary

Join our dedicated healthcare team as a Patient Care Technician (PCT) / Certified Nursing Assistant (CNA), where your compassionate care and clinical skills will make a meaningful difference in patients' lives. In this vital role, you will provide direct patient support across various healthcare settings, ensuring comfort, safety, and dignity while assisting with daily activities and essential medical procedures. Your energetic approach and attention to detail will help foster a positive environment for patients of all ages and needs, from acute care to long-term and community-based care.

Duties

Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility using safe lifting practices and equipment like Hoyer lifts.
Monitor vital signs including blood pressure, pulse, respiration, and temperature; document findings accurately in electronic health record (EHR) systems.
Support medication administration under supervision, ensuring proper handling and adherence to HIPAA regulations to maintain patient confidentiality.
Perform basic clinical procedures such as catheter care, phlebotomy, wound dressing changes, and aseptic techniques to promote healing and prevent infection.
Facilitate patient transfers using appropriate transfer methods and equipment; assist with positioning for comfort or medical procedures.
Communicate effectively with patients, families, and interdisciplinary team members to coordinate care plans and address individual needs.
Maintain a safe environment by adhering to infection control protocols, practicing universal precautions, and ensuring compliance with hospital or long-term care facility standards.

Qualifications

Current certification as a Nursing Assistant (CNA) or equivalent credential required; additional Patient Care Technician certification preferred.
Experience working in diverse healthcare environments such as hospitals, long-term care facilities, assisted living communities, or home health settings. Hospice care is preferred.
Knowledge of medical terminology, physiology, and patient care techniques including vital signs monitoring and medication administration.
Familiarity with electronic health records (Winnolas/EHR/Epic/eClinicalWorks/Cerner) systems for accurate documentation.
Demonstrated ability to work with individuals with disabilities or complex medical conditions such as dementia, Alzheimer's disease, or behavioral health needs.
Strong communication skills to effectively collaborate within multidisciplinary teams and provide compassionate patient service.
Physical ability to perform heavy lifting, manual handling tasks, and assist with patient transfers safely.
Commitment to maintaining patient confidentiality in accordance with HIPAA regulations; understanding of infection control practices including aseptic technique.
