Job Summary

We are seeking a dedicated and proactive Discharge Planner/Case Manager to join our dynamic healthcare team. In this vital role, you will coordinate patient discharges, facilitate seamless transitions of care, and collaborate with multidisciplinary teams to ensure optimal patient outcomes. Your expertise will help bridge the gap between inpatient care and community resources, promoting safe and efficient post-acute recovery. This position offers an exciting opportunity to make a meaningful difference in patients' lives while working in a fast-paced, compassionate environment.

Duties

Conduct comprehensive patient assessments to determine discharge needs, including medical, psychological, and social factors
Develop individualized discharge plans that incorporate community resources, and patient preferences
Collaborate closely with physicians, nurses, social workers, therapists, and other healthcare professionals to coordinate care transitions
Facilitate communication with patients and their families to explain discharge procedures
Manage electronic health records (EHR) accurately to document assessments, plans, and follow-up actions in compliance with HIPAA regulations
Coordinate with community agencies and service providers to arrange post-acute care services
Monitor patient progress post-discharge to ensure adherence to care plans and address any emerging needs or concerns

Skills

Proven experience working with diverse patient populations including Homeless, mental health conditions, or substance use disorders
Skilled in clinical case management, utilization management, and behavioral health coordination
Proficiency in EMR/EHR systems and familiarity with HIPAA compliance standards
Excellent client communication skills with the ability to work effectively with patients’ families and multidisciplinary teams
Ability to prioritize tasks efficiently in high-pressure environments
Knowledge of community resource coordination within health services social work frameworks
Experience working across inpatient, outpatient, or clinic environments is highly desirable
