HELLO, WE'RE COMMUNITY LIVING CASE MANAGEMENT

We support children, adults, and their families to access both in-home and community services. Once determined eligible, we utilize a Person-Centered approach addressing the health and safety needs for the individuals and families being served. We assist individuals and families to identify natural and community supports, potential supports in their own homes, and potential Medicaid-funded supports in the community. We encourage self-determination and offer a supported decision-making process for adults seeking services. Families with children seeking services will receive information on supports available within their communities.

ARE YOU EXCITED ABOUT THIS SERVICE COORDINATOR/CASE MANAGER JOB?

As a Service Coordinator at Community Living Case Management in The Dalles, you will play a vital role in managing and facilitating access to essential supportive services within the community. Your responsibilities will include providing comprehensive case management services tailored to individual needs, ensuring that all clients receive the support they require. You'll have the opportunity to develop innovative programs and resources that promote wellness for individuals with Intellectual and/or Developmental Disabilities, fostering an environment where they can thrive.

WOULD YOU BE A GREAT SERVICE COORDINATOR/CASE MANAGER?

To succeed as a Service Coordinator at Community Living Case Management, you will need strong communication and interpersonal skills, particularly proficiency in both English and Spanish, to effectively connect with a diverse range of clients and their families. Problem-solving abilities are crucial as you navigate complex situations and develop tailored solutions for individuals with Intellectual and/or Developmental Disabilities. Familiarity with community resources and supportive services will help you provide the best case management and program development opportunities to the clients you support.

Additionally, being empathetic and culturally aware will enable you to build trust within the community. A proactive approach, combined with organizational skills, will ensure that you manage multiple cases efficiently while consistently delivering high-quality support. This position requires a commitment to excellence and integrity, making you an integral part of our mission to empower individuals in need.

Knowledge and skills required for the position are:
