Clinical Infusion Pharmacist
Full-Time | Evening Shift
Position Summary
The Clinical Infusion Pharmacist is responsible for the clinical review, sterile compounding, verification, and dispensing of patient-specific infusion and specialty medications. This pharmacist plays a central role in ensuring that all preparations are accurate, sterile, stable, compatible, appropriately labeled, and dispensed in accordance with prescriber orders, applicable regulations, accreditation standards, and company policies.
The Clinical Infusion Pharmacist works closely with prescribers, nursing agencies, patients, caregivers, pharmacy technicians, intake personnel, and other members of the patient-care team. Responsibilities include evaluating medication orders, performing clinical and pharmaceutical calculations, overseeing sterile compounding activities, monitoring patient therapy, resolving clinical concerns, and supporting the pharmacy’s quality-management program.
The pharmacist may supervise pharmacy technicians and other pharmacy support personnel and may assist with staff training, competency assessments, quality assurance, and performance-improvement activities.
This position reports to the Director of Pharmacy. When designated and appropriately qualified, the Clinical Infusion Pharmacist may provide administrative or operational coverage in the absence of pharmacy leadership.
Essential Duties and Responsibilities Clinical Review and Patient Care
- Reviews and evaluates infusion prescriptions and physician orders for completeness, accuracy, clinical appropriateness, and regulatory compliance.
- Verifies patient identity, allergies, medication history, diagnoses, laboratory results, administration method, infusion device information, treatment schedules, refill dates, prescriber information, and other relevant clinical data.
- Reviews prescribed medications, concentrations, total volumes, doses, dosing changes, administration rates, treatment frequency, and duration of therapy.
- Performs and independently verifies pharmaceutical calculations involving concentrations, dilutions, displacement, total drug quantities, infusion rates, and final preparation volumes.
- Evaluates medication compatibility, stability, beyond-use dating, storage requirements, and administration considerations using appropriate clinical references and available supporting data.
- Identifies and resolves medication-related concerns, including unusual doses, concentration discrepancies, duplicate therapy, incompatibilities, stability concerns, allergies, potential drug interactions, adverse effects, or unclear orders.
- Communicates with prescribers and other members of the patient-care team to clarify orders and recommend clinically appropriate changes when necessary.
- Reviews relevant laboratory results and patient assessments and makes appropriate recommendations to support safe and effective therapy.
- Documents clinical interventions, prescriber communications, therapy changes, adverse reactions, and other relevant patient-care activities.
- Serves as a clinical resource regarding infusion medications, specialty medications, administration devices, supplies, storage requirements, handling procedures, and medication safety.
- Coordinates with nursing agencies and other healthcare professionals regarding patient care, medication delivery, treatment scheduling, clinical assessment, and continuity of therapy.
- Provides medication counseling and education to patients and caregivers when appropriate.
- Participates in adverse drug reaction reporting, medication-error review, and other patient-safety activities.
- Maintains patient confidentiality and protects health information in accordance with HIPAA and company policy.
Sterile Compounding and Dispensing
- Compounds or oversees the compounding of patient-specific sterile preparations using proper aseptic technique.
- Performs final pharmacist verification of compounded sterile preparations before dispensing.
- Ensures that each preparation matches the authorized prescription and is accurately compounded, labeled, packaged, stored, and transported.
- Verifies ingredients, lot numbers, expiration dates, calculations, preparation volumes, final concentrations, labels, ancillary supplies, and required documentation.
- Maintains working knowledge of USP General Chapter <797>, applicable provisions of USP <800>, Louisiana Board of Pharmacy requirements, federal regulations, accreditation standards, and company policies.
- Follows established procedures for garbing, hand hygiene, aseptic manipulation, cleaning, disinfection, material handling, and movement within controlled compounding areas.
- Assists with maintaining clean, organized, and compliant pharmacy and sterile compounding work areas.
- Completes and maintains required sterile compounding training and competency assessments, including those related to garbing, hand hygiene, aseptic technique, media-fill testing, and fingertip sampling.
- Ensures that compounded preparations are quarantined, released, or rejected in accordance with established policies and applicable quality-control requirements.
- Recognizes and reports compounding deviations, damaged products, temperature excursions, sterility concerns, labeling discrepancies, and other potential quality issues.
- Supports the proper handling, documentation, accountability, reconciliation, and security of controlled substances and other high-alert medications.
- Participates in inventory management and helps minimize medication waste while maintaining adequate product availability.
- Ensures medications and supplies are appropriately packaged and prepared for safe and timely delivery.
Quality Assurance and Regulatory Compliance
- Supports the pharmacy’s quality assurance, quality control, and performance-improvement programs.
- Participates in the review and investigation of medication errors, compounding deviations, patient complaints, adverse events, sterility concerns, and other reportable incidents.
- Assists with corrective and preventive actions and monitors their effectiveness when assigned.
- Maintains complete and accurate documentation of prescriptions, compounding activities, dispensing records, clinical interventions, controlled substances, and patient-care services.
- Follows company policies, pharmacy procedures, state and federal regulations, accreditation requirements, and professional standards of practice.
- Participates in inspections, audits, accreditation surveys, and regulatory reviews as requested.
- Remains current on infusion therapy, sterile compounding practices, clinical guidelines, regulatory requirements, accreditation standards, and industry developments.
- Identifies opportunities to improve medication safety, workflow efficiency, documentation, and patient outcomes.
- Reports quality, compliance, or patient-safety concerns promptly to pharmacy leadership.
Leadership and Team Support
- Provides direction and oversight to pharmacy technicians and support personnel working under the pharmacist’s supervision.
- Ensures that delegated activities are appropriate and performed by trained and competent personnel.
- Assists with employee orientation, training, competency assessment, coaching, and preceptorship.
- Promotes a culture of accountability, accuracy, patient safety, professionalism, teamwork, and continuous improvement.
- Collaborates with pharmacy leadership and other departments to address workflow, scheduling, patient-care, and operational needs.
- Participates in pharmacy meetings, clinical discussions, quality reviews, and staff-development activities.
- Provides coverage for other pharmacist responsibilities when necessary, provided those duties are consistent with the pharmacist’s licensure, training, competency, and applicable regulations.
- May provide administrative or operational pharmacy coverage when the Director of Pharmacy or designated manager is unavailable.
- Completes assigned documentation, reports, training, and other responsibilities accurately and within established deadlines.
- Performs other pharmacist duties reasonably related to the position and within the scope of professional practice.
Required Qualifications
- Doctor of Pharmacy degree or Bachelor of Science in Pharmacy from an accredited college or school of pharmacy.
- Active and unrestricted pharmacist license in good standing with the Louisiana Board of Pharmacy.
- Knowledge of pharmacy law, sterile compounding standards, medication safety, and professional pharmacy practice.
- Strong pharmaceutical calculation and clinical evaluation skills.
- Ability to communicate professionally and effectively with prescribers, nurses, patients, caregivers, referral sources, and coworkers.
- Strong attention to detail and the ability to work accurately with high-alert and patient-specific medications.
- Ability to organize competing priorities and complete time-sensitive work in a clinical production environment.
- Ability to maintain confidentiality and exercise sound professional judgment.
- Willingness and ability to complete all required sterile compounding training and competency assessments.
- Ability to work independently and collaboratively as part of a multidisciplinary healthcare team.
Preferred Qualifications
- Three or more years of clinical pharmacy experience.
- At least one year of infusion pharmacy or sterile compounding experience.
- Experience with infusion therapy, specialty pharmacy, pain management, parenteral nutrition, anti-infective therapy, biologic therapy, or other complex medication regimens.
- Experience with ambulatory or implanted infusion devices.
- Formal training in sterile compounding and aseptic technique.
- Experience supervising pharmacy technicians or other pharmacy support personnel.
- Experience working with USP <797>, USP <800>, accreditation requirements, and pharmacy quality-management programs.
- Experience working with controlled substances and high-alert medications.
- Additional pharmacist licenses in states serviced by the pharmacy.
- Board certification or other advanced clinical pharmacy credentials.

Schedule
This is a full-time position with flexible shift scheduling based on pharmacy operations and patient-care needs. The pharmacist may occasionally be asked to provide additional coverage based on staffing, workload, or continuity-of-care requirements. Any on-call, weekend, holiday, or after-hours expectations will be communicated as part of the assigned schedule.
Work Environment and Physical Requirements
- Work is primarily performed in a pharmacy, office, and controlled sterile compounding environment.
- Must be able to comply with all garbing, hand-hygiene, and personal protective equipment requirements.
- Must be able to remain seated or standing for extended periods while completing clinical, dispensing, or compounding responsibilities.
- Must possess the visual accuracy, manual dexterity, and attention to detail required for sterile compounding and final verification.
