Full Job Description

BASIC FUNCTIONS:

Under the supervision of the Senior Program Manager the successful candidate will be responsible for providing supervision for staff treating clients within the High-Fidelity Wrap-Around program which provides intensive care-coordination services to children, youth, and families in crisis in office, in-home, and in the community. High-Fidelity Wraparound is an evidenced-based care coordination process that has been demonstrated as an effective way to support children and youth with serious behavioral health challenges to live successfully in the community with their families.

SUPERVISION: Supervised by Senior Behavioral Health Program Manager

ESSENTIAL JOB FUNCTIONS:

1. Treat all clients with dignity and respect.

2. Provide evaluations/problem identification, crisis interventions; assess the need for on-going treatment or referral to other agencies.

3. Provide individual and group supervision to the High-Fidelity Wrap-Around Team.

4. Coach team on both HFW principals as well as MHBHC policy and procedures.

5. Take required training and credentialing steps by working with a coach to demonstrate competency and fidelity to the wraparound principles and action steps (must complete initial credentialing 12 months after hire and must complete re-credentialing every two years thereafter).

6. Collaborate with NWIC to ensure coaching through the workforce capacity center is consistent and addressing team need.

7. Collaborate with the client’s team to develop, implement, and revise individualized transition plans, treatment plans and monitor treatment progress.

8. Coordinate care for clients with other professionals and referral partners.

9. Maintain accurate and timely clinical records consistent with BHA standards in all areas including, but not limited to: counselor admit notes, assessments, daily charting, weekly clinical staffing notes, and discharge summaries.

10. Develop weekly team meeting agendas & facilitate team meeting with direction from the Senior Program Manager

11. Conduct regular stakeholder meetings of key partners related to progress and implementation of grants

12. Collaborate with Medicaid RAE’s and the BHASO’s on referrals and capacity needs.

13. Ensure consistency in filing of paperwork across all grants, including grant reports.

14. Spot-check documentation for accuracy and completion

15. Observe confidentiality requirements.

16. Perform other duties as assigned.

QUALIFICATIONS:

Desired Knowledge & Skills

· Familiarity with recovery of Substance Abuse and Mental Health Disorders

· Broad base of experience with life and possess a diverse view of what families need to have better lives

· Knowledge of trauma informed care

· Knowledge of local agencies’ programs and services (schools, social services, law enforcement, courts/corrections), local units of government, and grassroots organizations

· Knowledge of funding systems and care providers for youth with identified mental health, developmental disability, substance use, and/or other acute needs.

· Possess strong communication and interpersonal skills

· Clearly communicate ideas in both written and verbal form

· Understand and relate to the needs of project participants of diverse ethnic, cultural, educational, and socioeconomic backgrounds

· Operate a computer to enter data using Microsoft office, maintain records through EHR, navigate data systems, and generate reports for funders and referral partners.

· Network effectively with various types of organizations, including government agencies, law enforcement agencies, schools, social service agencies, courts/probation/corrections, and grassroots organizations

· Work effectively with community residents, diverse population groups

· Exercise judgment regarding appropriate information sharing, confidentiality requirements, and human relations while setting clear boundaries

· Prioritize tasks and work independently in the absence of supervision

· Work effectively with agency staff and service providers and with individuals experiencing criminal justice involvement, incarceration, substance abuse and mental health needs.

· Ability to work a flexible, non-traditional schedule, including evenings and/or weekends.

· Ability to network effectively with various types of organizations, including government agencies, law enforcement agencies, schools, social service agencies, courts/probation/corrections, and grassroots organizations.

· Ability to exercise judgment regarding appropriate information sharing, confidentiality requirements, and human relations while setting clear boundaries.

· Ability to prioritize tasks and work independently in the absence of direct supervision.

EDUCATION AND EXPERIENCE:

Clinical Licensure as a LCSW, LPC, or LAC and registered successfully with DORA.
Education: Master’s degree in social work or related field preferred
Experience Preferred: Minimum of four years’ experience in working with youth and families.
1 year of experience managing people and providing supervision.

OTHER REQUIREMENTS:

Valid Driver’s License and car insurance by date of hire
Ability to pass background check

Pay: $70,000.00 - $85,000.00 per year
