Full Job Description

The East Toledo Family Center (ETFC) is the anchor organization in the East Toledo community. We provide programs and services to support individuals and families from birth to seniors. We educate, engage and empower our families and community to make their own changes. The Pathways Community Health Workers (CHW) are front line public health workers who have a close understanding of the community they serve. This trusting relationship enables them to serve as a liaison between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.

PRINCIPLE DUTIES & RESPONSIBILITIES:

· Conduct home visits with pregnant and adult clients to provide education, support, and care coordination

· Develop and maintain trusting relationships to engage clients in voluntary services

· Maintain an active caseload of 30+ clients while ensuring consistent follow-up and engagement

· Complete ongoing client assessments using required program tools and visit forms

· Provide health education using approved program materials and evidence-informed resources

· Support clients in developing and achieving self-identified goals related to health, parenting, and stability

· Connect clients to medical, social, and community resources to strengthen family outcomes

· Complete accurate and timely documentation in the program database within 48 hours of each client interaction

· Participate in regular team meetings and bi-weekly supervision to review cases, interventions, and documentation

· Collaborate with team members and community partners to support coordinated care

· Assist with outreach activities, including community events and fairs, to promote program awareness

· Complete all required training and maintain compliance with program and agency expectations

PHYSICAL REQUIREMENTS:

Pass all required background and drug tests
Locations - office environment and diverse community client homes
Must be able to provide in-home visits
Valid driver’s license
Travel Requirements- Must have reliable vehicle, ability to travel to and from family homes with insurance requirements listed below (mileage is reimbursed at the state rate)
Must maintain a valid driver’s license and personal automobile insurance meeting minimum liability requirements of $100,000 per person/$300,000 per accident for bodily injury and $50,000 per accident for property damage, in accordance with agency policy. Proof of coverage is required.

EDUCATION:

High School Diploma or equivalent
Associates Degree preferred but not required
Community Health Worker Certification preferred but not required
Individuals who are not certified community health workers will have the opportunity to obtain their certification through ETFC.

An ability to establish trusting relationships
Acceptance of individual differences
Preferred experience and willingness to work with culturally diverse populations
Preferred knowledge of prenatal, infant and child development
Strong verbal and written communication skills
Ability to maintain professional boundaries when providing supportive services
Ability to work collaboratively and independently with work schedule

PAY RANGE: $16.00 – 18.00 hr.

SCHEDULE: Full-Time, 8-hour shifts, Monday through Friday.

TRAINING: Orientation to the Family Center Employee Handbook. All job-related training is required.

UNIFORM: Staff badge and business casual attire required.

BENEFITS: 403(b), Health, dental, and vision insurance, 10 paid holidays, and paid time off after a year.

Pay: $16.00 - $18.00 per hour
