Full Job Description

As the Foster Care Case Manager (FCCM), you will be responsible for safety and services to children and families in foster care and adoptive homes. Tasks include coordination of individualized services across all settings, coordination of providers working in the home, school, and community environment. The Foster Care Case Manager will maintain all files and documentation of support related activities and services in accordance with Medicaid, DCFS/CPS, etc. The Foster Care Case Manager promotes effective utilization of support services to achieve permanency for children and families.

Responsibilities/Essential functions: The person in this position must be able to perform the following essential job functions with or without reasonable accommodations.

Must travel to office Monday - Friday, located in North Houston TX 77060 (Greenspoint/IAH Area)
Providing therapeutically based direct client service delivery: placement, assessment and supervision of children in foster and adoptive homes, foster and adoptive parent support and supervision, case management, service planning, permanency planning, risk assessment and collaboration with other service providers.
Maintaining client and foster and adoptive case history and home records/reports to meet CQI and minimum standards, providing documentation of all medical, dental, therapeutic, evaluation reports, providing appropriate reports to CPS.
Records progress related to rehabilitation goals through home visits with the child and the foster parents. Revises goals as necessary.
Responsible for required home visits and monitoring services each month for all assigned cases, including behavioral health, attendance, court hearings, school staffings, biological family visits, behavioral health appointments, psychiatric staffings, team meetings and any other case management activities.
Always Ensure the proper supervision of children/youth; serve as an advocate and resource for youth, foster parents, and agency staff
Train clients in a variety of coping skills utilizing standardized curriculum.
Teach/train natural, foster, and adoptive parents standardized parenting skills as well as individualized techniques for working within their family.
Monitor treatment service program files for compliance to ensure standard compliance through training and/or necessary corrective action.
Work collaboratively with relevant state departments to provide services to each youth and family as specified by the state Plan of Care and the treatment plan.
Develop and maintains a referral process and source of community partners to facilitate link to resources.
Attend all departmental and program meetings to ensure that up-to-date information is received and/or information regarding policy changes or practice are adhered to.
Attend training that will enhance professional growth as Case Manager or other topics as deemed appropriate.
Other duties as assigned

Qualifications Requirement:

Bachelor’s degree (B.A. or B.S.) from four-year college or university in a social service related field required
3+ years of case management experience required
At least one year of experience in a social service agency or child-welfare related experience and/or training preferred.
Must be proficient in use of the computer: word processing, data entry, and designing and creating spread sheets and data tables/charts
Must be self-directed, organized, detail oriented and possess excellent written and verbal communication skills
Must be able to follow instructions, follow through, and maintain timeliness.
Must have thorough understanding and commitment to timeliness and have a sense of urgency in meeting the needs of the children
Must be willing to travel to various service providers, offices of the agency or general transportation of youth
Ensure strict confidentiality of client records according to policy and procedures adhering to HIPAA.
Knowledge of relevant policies, procedures, and regulations pertaining to the release of confidential information.
Must possess reliable transportation, a State of Texas Driver’s License, valid vehicle insurance and vehicle registration.
Subject to work extended hours, weekends, and/or on-call.

Schedule:

In Office: Monday to Friday 9am to 4pm / Tuesday 8am to 4pm
Located in North Houston, TX 77060 (Greenspoint/IAH Area)
