Temporary Part Time Social Worker

Pay: $26.66 per hour

Location: Winnetka

Shift: Mon-Fri 7:30am-2:30pm

Employee Type: Temporary/Part Time

Momemtum Valley Village is a nonprofit organization providing services to adults with developmental disabilities. We are currently seeking an Temporary Part Time Social Worker to work at our Winnetka Adult Day Health Center.

This is a temporary part-time non-exempt position. The Social Worker will work 16 hours a week or 2 days. The 16 hours/2 days will be covering shifts on Tuesday and Thursday from 7:30am-3:00 pm, additional days or hours may be required based on the needs of the organization.

Job Summary:

JOB SUMMARY:

This is a non-exempt position. The Social Worker will coordinate the efforts of the multidisciplinary team and take an overall role in ensuring that each participant’s needs are being addressed.

Job Duties:

· Schedule and chair multidisciplinary meetings in conjunction with the program director.

· Interview and screen all referrals to determine general appropriateness of the perspective participant for the full assessment process and adult day health care participation.

· Provide referral for persons not appropriate for the program.

· Provide a signed and dated documentation for all services performed the day the service is provided and include signed and dated quarterly progress reports in each participant record.

· Provide for periodic reevaluation and revision of plan of care.

· Provide counseling and referral to available community resources.

· Promote peer-group interaction and relationships through discussion groups.

· Serve as liaison with the participant’s family and home.

· Serve as liaison with other community agencies that may be providing services and coordinate all services to a participant and work with these agencies to coordinate all services delivered to the participant to meet the participant’s needs and avoid duplication.

· Provide discharge planning for all discharge participants.

· Monitor the work of program consultants.

· Staff training.

· Supervise and train assistant social workers.

ACCOUNTABILITIES:

· Since you are a mandated reporter, if you see any type of abuse happening, you must report it immediately.

· Recognizes emergency situations and the appropriate action to take.

· Performs other tasks as assigned by the Program Director or Administrator.

· Participation in scheduled department and all staff meetings.

· CPR class must be taken yearly and First Aid every two years. You are responsible for maintaining your training up to date.

· Physical examinations and Chest X-ray or TB test are to be taken every year.

QUALIFICATIONS:

· Master’s degree in social work from an accredited school of social work or be licensed as a clinical social worker with the California Board of Behavior Science Examiner.

· At least 3 years of experience working in a licensed health care setting.

· Must be able to comprehend and communicate in English.

· Must meet all insurance requirements which include a valid driver’s license with a good driving record, criminal record clearance, etc.

· Must maintain the highest standards of appearance (personal grooming and dress) and set an appropriate model for the participants.
