POSITION DESCRIPTION

POSITION TITLE: Case Manager

SALARY LEVEL: $22 an hour

BENEFITS: Medical, Dental, Vision, Life insurance Vacation/Sick/PTO Accrual

DEPARTMENT: Transitional Shelter

REPORTS TO: Shelter Manager

DESIGNATION: Full Time

HOURS: 8 A.M. to 5 P.M., Monday – Friday

Hours may vary according to agency needs.

POSITION SUMMARY: Under the direction of the Transitional shelter Manager, the Case Manager participates in all programs and activities for staff and clients. The Case Manager empowers survivors by following the agency’s mission and goals. The Case Manager supports the Program Manager to ensure the success of the Shelter Program.

PRIMARY POSITION PRINCIPLES:

· Assisting with program administration, the training of staff and representation of the program internally and externally

· Ensuring that evidence-based practices are utilized through example and by providing training

· Conducting individualized needs assessments and collaborating with clients to develop permanent housing plans that address any and all barriers to obtaining and maintaining stable housing

· Providing on-going case management support to assess progress and ensure housing plan goals are met or adjusted as necessary

· Coordinate the integration of social services/ case management functions into the participates planning process.

· Conduct regularly review of all participants file from all the program.

· Keeps detailed records of every case, every referral and every contact with other agencies on behalf of the clients.

· Help clients work through urgent needs. Enable clients to take responsibility for their recovery, acting as an advocate and a facilitator. Work in partnership with survivors.

· Relay all information to Shelter Manager on a weekly basis and keep detailed records of these meetings.

· Network with other agencies to stay informed about services and resources. Refer clients to another program when appropriate.

Follow up with other agencies that have made commitments to assist clients with their needs

· Respect confidentiality at all times, in public, committee meetings, consultations, as well as during off-duty hours outside of work.

· Remain in contact with client until the client’s needs are met and the case is closed.

DUTIES AND RESPONSIBILITIES:

Referrals and linkage to community providers for health, mental health, and substance use services
Admit, process, and provide an orientation to new clients utilizing agency intake procedures.
Carry a caseload of 4-8 clients assigned by the Shelter Manager
Make appropriate referrals and assist clients with their exit planning.
Maintain appropriate documentation for all services provided.
Monitor client adherence to all shelter procedures and house rules.
Distribute clothing, personal items, and medication to clients as needed.
Assist with receipt, organization, and distribution of donations.
Responsible for ensuring the cleanliness, orderliness, safety and sanitation of the shelter through supervision of residents in their household duties.
Represent the agency at community meetings and special events as assigned, such as councils, chambers of commerce, etc.
Additional duties and special projects may be assigned as necessary.

MINIMUM QUALIFICATIONS:

1. Certification in the 40-hour domestic violence victim services training required by the State of California or willing to obtain within first 3 months of employment.

2. Bilingual required in English/Spanish.

3. Good written and oral communications skills.

4. Possess basic computer knowledge and skills.

5. Clear state and Department of Justice criminal background check.
