Full Job Description

Windham is looking for a proactive, organized, personable, articulate, fast thinking and tech savvy individual to join our team! The Telephonic Nurse Case Manager will work as a liaison between the employer, doctors, adjuster and claimant for each Workers Compensation Case. Our approach is to help the client return to work while also managing their recovery and ensuring cost containment.

Location: California

The Telephonic Nurse Case Manager will work with attention to detail and provide case management to promote efficient coordination of client care through collaborative practice within the healthcare systems and multiple environments.

Essential Responsibilities:

Assess, plan, implement, coordinate, advocate, monitor and evaluate treatment plans by physicians/healthcare teams while ensuring cost containment and return to work is addressed proactively.
Establish positive relationships with the claimant, employer, TPA’s, carriers, attorneys, community resources, physicians and other health care providers.
Provide carriers with concurrently reviewed information as needed to comply with their requirements for documentation of medical necessity and negotiate resolution of any disagreement over the need for services.
Coordinate proactive solutions to overcome barriers with recovery and return to work.
Document all interactions and outcomes related to claim to include monthly reporting.
Maintain the dignity and confidentiality of the client and all verbal/written information/ communication.
Provide education for client and family members regarding the Worker’s Compensation system, members of the healthcare team on managed care requirements and their implications, and patient care and health care providers on the concept, process and goals of case management.

Knowledge Skills & Abilities:

Strong communication skills; oral and written
Must be able to demonstrate team behavior and must be willing to promote a team-oriented environment
Ability to multi-task, have proficient time management skills, and ability to work independently
Strong facilitation and team building skills
Ability to interact with multidisciplinary teams in an assertive yet sensitive manner.

Must-Haves, No exceptions:

Excellent technology skills with Microsoft Word, medical records systems, smart phones
Graduate of an accredited school of nursing
RN is required, CCM preferred

Licensure Requirement:

Current, active, and unencumbered license as a Registered Nurse in California

Desired Nice-to-Haves:

Emergency Room, Home Health and/or Orthopedic experience preferred
Knowledge of case management, workers’ compensation, managed care, or ergonomics
