Role Summary
The Care Manager/“Designated Care Manager” is responsible for providing the highest degree of quality care and services to a
consistent group of residents and their families in our assisted living and reminiscence neighborhoods. Th e Care Manager
provides hands on care and physical and emotional support as outlined in each resident’s Individualized Service Plan (ISP)
while maintaining a safe and comfortable home like environment. The Care Manager/“Designated Care Manager” is
responsible for demonstrating the Mission for Sunrise Senior Living, “to champion quality of life for all seniors” in accordance
with federal, state, and local laws and regulations and Sunrise Senior Living standards, policies, and procedures to promote the
highest degree of quality care and services to our residents.

Resident Car e:
- Reviews, reads, notates, and initializes Daily Log to document and learn about pertinent information about
residents.
- Participates in the development of the ISPs and monthly updates.
- Reviews designated assignments.
- Responsible for a designated groupofresidents during the shift, knows where residentsare, and physically checks
on them throughout the shift.
- Observes,reports, and documentssymptoms and conditions of residents for changes in condition such as skin,
behavior, alertness, weight, diet, and participation in activities.
- Attends daily Cross Over meetings by the lead care manager.
- Notifies supervisor and resident care director if a resident has increased care needs.
- Informs supervisor of any resident changes in condition.
- Responds to security system and resident call bells promptly and immediately. Takes appropriate action including
resetting call bells.

- Greets guests, family members, residents, and team members.
- Answers, directs and/or responds to phone calls in a timely, courteous, and professional manner.
- Assists with continence management and disposes of all continence products properly to ensure sanitation of
resident suite and community restrooms using standard care procedures.
- Communicates with families and is a resource as needed.
- Helpsresidents maintain independence and physicalsafety andpromotes dignityof each resident adhering to the
standards of Resident Rights and Sunrise Principles of Service.
- Actively participates/leads and assists residents with activities of daily living (ADLs) and Invites, Encourages, and
Assists (IEA) residents to participate in activities as indicated on the ISP.
- Engages residents in life skills and other life enrichment activities throughout the day in reminiscence.
- Strives to understand and respond to each resident with empathy, always remaining mindful of the
resident’s unique communication patterns or history and basic human needs.
- Maintains an atmosphere of warmth, personal interest, and positive emphasis as well as a calm
environment.
- Ensures the established safety regulations are always followed.
- Practices routinely good Standard Care Precautions of cleanliness, hygiene, and health.
- Hosts and engages in activities with the residents daily.
Risk Management and Gener al Safety:
- Partnerswith community teamto ensure community is in compliance with OSHArequirements, promoting
Risk Management programs and policies, and adhering to safety rules and regulations.
- Practicessafety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, Safety
Data Sheets (SDS), and Lockout Tagout procedures.
- Reports all accidents/incidents immediately.
- Reports all unsafe and hazardous conditions/equipment immediately.
- Ensures any cords, carts, equipment, and otherhazards are always kept out of the way, not blocking exits, and in
compliance with fire codes.
- Understands and practices the proper method of attending to and disposing of, and the possibility of exposure to,
blood borne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials.
- Reports occupational exposures to blood, body fluids, infectious materials, sharp sticks, and hazardous
chemicals immediately.
- Ensures oxygen tanks arestoredsafely, exchangedwhen empty and monitored to make sure liters of oxygen are
at prescribed levels.
Housekeeping and Laundr y Ser vices:
- Maintains and cleansresident’s room as assigned including making beds, emptying trash, putting clothes away
appropriately, spot sweeping and mopping as needed.
- Washes resident’s laundry as noted in the ISP and as needed.
- Washes and folds dining room linens and napkins.
- Completes assigned housekeeping tasks.
- Maintains common areas in a clean and tidy manner at all times.
Dining Ser vice:
- Serves meals in the dining room and works in the dining room as assigned.
- Promotes and ensures a pleasant dining experience during all meals.
- Assists with dining room set up and clean up as assigned.
- Participates in pre-meal meetings.
- Follows residents’ diets as indicated on ISP and Confidential Diet Board/Chart.
- Observes, notes, and documents in daily log any resident changes in dining habits.
- Responds to the dining needs of the residents and guests while maintaining Sunrise hospitality and service
standards.
- Provides room service delivery as needed.
- Practices safe food handling in compliance with universal care precautions at all times.
Specific Responsibilities for the Reminiscence Neighbor hood Car e Manager:
- IEA residents in life skills and other life enriching activities as indicated on ISP and demographic profile.
- IEA residents to attend the afternoon social.

- Incorporates the concept of ‘Creating Pleasant Days’ into the resident’s daily routine.
- Integrates the individual resident’s life skills into their daily routine.
- Blends a variety of multi-sensory experiences into the resident’s day.
- Actively participates in monthly letters home and letter writing with the residents and their families.
Tr aining and Contr ibuting to Team Success:

- Attends regular meetings; Town Hall, Department Team, Cross Over, and others as directed by the
Supervisor/Department Coordinator.
- Attendsregular training by Assisted Living Coordinator (ALC), Resident Care Coordinator (RCD) or Reminiscence
Coordinator (RC) using mini modules.
- Complieswith all infectioncontrol techniques, placement of bio-hazard containers, and removal techniques as
listed in policies and procedures.
- Maintains compliance in assigned required training and all training required by stateor other regulatingauthorities as
applicable to this role to ensure that Sunrise standards are always met.
Experience and Qualifications
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements

- High School diploma/GED accepted and may be required per state regulations.
- CPR Certificate and First Aid as required by state regulations.
- Must be at least 18 years of age.
- Previous experience working with seniors preferred.
- Desire to serve and care for seniors.
- Ability to make choices and decisions and act in the resident’s best interest.
- Ability to react and remain calm in difficult situations.
- Ability to handle multiple priorities.
- Possess written and verbal skills for effective communication and a level of understanding.
- Competent in organizational and time management skills.
- Demonstrates good judgment, problem solving and decision-making skills.
- As applicable, all Sunrise team members who drive a Sunrise vehicle must sign the Driver job description and
understand the key essential duties for safety and regulatory compliance.
