Full Job Description

Benefits: 200 hours of paid annual PTO, 10 paid annual holidays, access to 401/k retirement, and paid medical/dental/vision.

Position Summary

This position provides case management and crisis support to youth.

Case Management:

Develop and maintain relationships with youth and proctors.
Develop and maintain positive working relationships and consistent communication with internal and external professionals involved in all stated domains of youths’ care and treatment.
Facilitate all aspects of case management from intake to transition planning, discharge, and aftercare when appropriate; maintain documentation in youth charts in accordance with YUI and BRS standards.
Create and implement Initial Service Plans and Master Service Plans (MSP) within designated time frames after the youth’s placement; provide routine MSP updates to the plan every 30-90 days, depending on service level. Manage all treatment documentation and its compliance requirements.
Generate an Assessment and Evaluation Report (AER) within 30-45 days of the youth’s placement, utilizing available resources and information as a foundation for service planning.
Coordinate and facilitate MSP meetings per BRS standards with youths’ teams to update progress and identify current needs, as well as attend pertinent meetings coordinated by external providers.
Utilize Collaborative Problem Solving as the primary behavior management approach, and support its implementation within the YUI spectrum of services.
Make referrals and advocate for youth and their families to ensure they are enrolled in services appropriate to their needs in all stated domains of youths’ care and treatment, including education/vocation. Coordinate family visits as appropriate.
Provides after-hours/weekend on-call crisis intervention on a weekly rotational basis; responds to situations that arise accordingly and within protocols, including required incident reporting documentation.
Develop rapport and working relationships with assigned YUI proctor families to provide supervision and support, ensuring alignment with youth service goals and compliance with regulatory expectations.
Collect, review, provide feedback, and file youth and proctor parents’ monthly paperwork in accordance with the YUI compliance schedule.
Participate in the development and implementation of proctor parent training, identifying topics that are relevant, informative, and engaging.
In collaboration with the YUI Foster Parent Certifier, Family Coordinator, and the social services team, ensure that any corrective actions for proctor parents are facilitated and monitored as needed.

Child Safety:

Monthly Home Health and Safety Checks (HHSC-BINTI)
Monthly foster parent supervision and skills training

General Responsibilities:

Maintain regular and reliable on-time attendance.
Participate in staff training, supervision, and staff meetings.
Maintain a calendar for work activities.
Some evening and occasional weekend work required.

Minimum Qualifications

Knowledge, Skills, and Abilities:

Social service staff shall have a master’s degree from an accredited college or university with major study in social work or a closely allied field and one year of experience in the care and treatment of children; or have a bachelor’s degree with major study in social work, psychology, sociology or a closely allied field, and two years of experience in the care and rehabilitation of children.
Must have a QMHA or be QMHA registered with the Mental Health and Addiction Certification Board of Oregon (MHACBO) and be able to obtain certification within 6 months of employment.
Knowledge of foster agency and foster parent licensing and compliance requirements
Knowledge of behavioral and health services systems throughout Oregon, and behavioral health policy rules in Oregon for Behavioral Rehabilitation Services OARs 410-170-000 to 410-170-0120, DHS Child Welfare Program OARs 413-090-0055 to 413-090-0090, and Medicaid Reimbursement OARs 309-172-0600 to 309-172-0860
Knowledge of HIPAA health privacy regulations and compliance requirements per 45 CFR Parts 160, 162, and 164
Knowledge of the training curriculum to provide training to foster care providers
Ability to prepare and present high-quality training and presentations for all key stakeholders (i.e., governmental and legislative, community-based organizations, funders, proctor parents)

Other Requirements:

Oral and written proficiency in the English language
The ability to drive a motor vehicle and provide one's own reliable transportation for the transport of oneself or others for business purposes (YUI provides business mileage reimbursement)
Computer literacy, including working knowledge of Microsoft Office Systems, Word, Google applications, and Outlook applications
Physical demands: sufficient physical ability and mobility to work in an office setting and community settings; to stand or sit for prolonged periods of time; to drive, to occasionally stoop, bend, kneel, couch, reach, and twist: to lift, carry, push, and/or pull light to moderate amount of weight; to operate office equipment requiring repetitive hand movement and fine coordination including use of a computer keyboard
Emergency Safety Response: Ability to competently and safely implement verbal and physical components as required by the Crisis Prevention Institute (CPI), as well as First Aid and CPR

Legal Requirements:

21 years of age
CPR, First Aid, and Crisis Prevention Institute (CPI).
Oregon Driver’s License
Proof of Insurance for vehicle liability coverage of $100,000/$300,000 personal damage liability, and $50,000 property damage liability.
Pass the ODHS Background Check
