Job Summary:

VNA of Albany is seeking a strong and dedicated Intake Specialist to join our team. As an Intake Specialist, you will play a vital role in managing the incoming patient referral process, verify insurance coverage and eligibility, and coordinate the transition of care from hospitals or physicians to home-based services. Our Intake specialist serves as the central link between referral sources, patients, and the agency’s clinical team.

Duties/Responsibilities:

· Review incoming referrals (electronic, fax, phone) and screen for admission eligibility.

· Verify insurance eligibility, reimbursement, and obtain prior authorizations as needed.

· Coordinate with hospital home care coordinators and referral sources to gather required documentation.

· Enter and maintain accurate patient records in EPIC/Focura.

· Track referrals and ensure timely processing by end of business day.

· Obtain and clarify patient admission information, including Face-to-Face (F2F) documentation for Medicare PDGM referrals.

· Escalate complex cases to the RN Intake Supervisor.

· Educate referral sources on admission criteria and documentation requirements.

· Build and maintain positive relationships with hospital discharge planners and referral partners.

· Respond to service inquiries and manage referral line communications.

· Maintain HIPAA compliance and protect patient confidentiality.

· Perform additional duties as assigned.

Requirements:

Associate degree in business or related field with at least two (2) years in healthcare.
Minimum one year of customer service experience required, preferably in the healthcare industry (i.e. hospital, long-term care, home care setting, or medical office)
At least 1-year of experience with intake/admissions.
Excellent computer skills required.
Prior use of Electronic Medical Records (EMR), (Highly Preferred)
Must have excellent communication and customer service skills.
Must work well independently and in a team environment.
Must be detail-oriented and flexible to changes.
Must have strong follow up skills.
Able to multi-task under time pressure.
