Occupational Therapist – Home Health

Location: Illinois – Home Health (Columbia, Dupo, Belleville, Swansea, O'Fallon, Red Bud, Collinsville, and more)

As a Home Health Occupational Therapist with EverTrue, you'll work one-on-one with patients in their homes, helping them improve their ability to safely complete everyday activities, maintain independence, and live life to the fullest.

As a PRN Occupational Therapist, you'll have the flexibility to build a schedule around your availability while providing skilled therapy to older adults in their homes.

What You'll Do

Evaluate patients and develop individualized occupational therapy treatment plans.
Provide skilled occupational therapy services in patients' homes.
Help patients improve safety, mobility, independence, and ability to complete daily activities.
Collaborate with physicians, nurses, physical therapists, speech therapists, social workers, and other members of the care team.
Educate patients, caregivers, and families on therapy plans and strategies they can use at home.
Supervise Occupational Therapy Assistants (COTAs) and evaluate their services and progress.
Establish goals and discharge plans based on each patient's needs.
Recommend and/or supervise ordering of specialized durable medical equipment when needed.
Complete timely and accurate documentation.
Communicate patient progress, changes in condition, and additional needs with physicians and the interdisciplinary team.
Participate in case conferences and agency in-service programs.
Rotate weekend and holiday on-call responsibilities.

Qualifications

Current Illinois Occupational Therapist license
Current CPR certification
Ability to work independently and manage your schedule
Excellent communication and customer service skills
Strong organizational and problem-solving skills
Basic computer and documentation skills
Home health experience preferred, but not required

Physical Requirements

This position requires standing, walking, climbing stairs, bending, lifting, transferring, and physically assisting patients in their homes while using safe body mechanics.

Pay

Our Home Health Occupational Therapists are paid using an episodic pay model.

Examples include:

Routine Visit: $50
Start of Care / Resumption of Care: $130
Weekend SOC / ROC: $140
Holiday SOC / ROC: $195
Evaluation Visit: $75
Weekend Evaluation Visit: $85
Holiday Evaluation Visit: $112.50
Recertification Visit: $100
Weekend and holiday rates available for many visit types.
Additional compensation for case conferences, quarterly meetings, compliance training, and other required activities.

A full episodic pay schedule will be reviewed during the interview process.

Eligible for shift differentials: No.
