Since 2014, SMC is an opioid treatment program (OTP) that delivers medically supervised services for adults through a variety of modalities, including individualized treatment, integrating pharmacological, medical, clinical and recovery support and case management services. SMC’s goal is to address the physical, emotional, and mental aspects of opioid and all substance use disorders, to help each of our patients achieve long-term recovery and an improved quality of life. We are looking for experienced Case Managers with Peer Support Certification to help guide others through their recovery from opioid and other illicit substance addictions. Your experience will be key in assisting clients to overcome the behavioral health challenges associated with the opioid crisis.
JOB SUMMARY
This case manager position is responsible for managing an assigned caseload of adults diagnosed with mental health and substance use disorders. Peer Support Certification required. You will be stationed at the Pima Reentry Center (PRC) located at 1275 W Starr Pass Blvd Tucson, AZ 85713, focused on helping those individuals who have been incarcerated. Applicants cannot be on parole or probation due to having to work in the PRC. The Case Manager is responsible for assessing client needs, developing, implementing, and reviewing treatment plans and discharge planning, and working directly with SMC clinical and medical staff and other community resources in meeting and achieving client needs

JOB RESPONSIBILITIES
- Responsible for managing an assigned caseload of adults diagnosed with substance use disorders.
- Provide individual and group therapy services at regularly scheduled intervals with all required documentation related to the client.
- Complete new client intakes, treatment plans, aftercare plans and discharge plans.
- Administer all required assessments as assigned and in accordance with SMC Policies and Procedures.
- Demonstrate appropriate and complete documentation of treatment in EHR within required timeframe.
- Coordinate services with outside providers including health care providers, probation/parole officers and DCS case workers.
- Identify case management needs related to social determinants of health (SDOH) and can include education, housing, employment and family and social environments.
- Facilitate educational group sessions on topics relevant to relapse prevention, life skills and other topics related to recovery.
- Involve families and significant others in patient’s recovery process when appropriate.
- Participate in regular staff meetings.
- Participate in regular individual and/or group supervision to enhance professional growth, review the provision of clinical/case management services, and address administrative challenges.
- Maintain professional development through Relias and complete all assigned training.
- Administrative duties related to the client to include gathering medical records, faxing, scanning, data entry and uploading documents to EHR.
- Perform other duties as assigned by supervisor, which are consistent with the position and in compliance with SMC policies and procedures.
MINIMUM QUALIFICATIONS
- A minimum of one-year experience providing case management services.
- A minimum of one year of experience in the substance abuse field.
- Unrestricted Driver’s License, verification of appropriate automobile insurance.
- AZDPS Level 1 Fingerprint Clearance Card.
- Current CPR/First Aid Certification.
- Negative TB Screening (within 1 year)
- Additional Preferred Certifications:
- Some college education (a plus) in the behavioral sciences or substance abuse.
- Experience and skills in working in substance abuse/behavioral health treatment programs.
KNOWLEDGE, ABILITIES, AND ATTITUDES REQUIRED
- Knowledge of software programs for personal computer use, especially word processing, email, spreadsheets and electronic medical records (EMR).
- Knowledge of case management resourcing, specifically with substance abuse clients.
- Ability to prepare, implement, and analyze treatment plans and psychiatric/medical reports.
- Strong problem-solving skills and the ability to carry out assigned projects to their completion.
- Strong interpersonal skills, including the ability to communicate effectively both verbally and in writing, establishing and maintaining effective working relationships, gaining cooperation and resolving conflicts.
- Ability to plan and organize case workload, manage deadlines of themselves.
- Ability to maintain and demonstrate ethical professional standards and demeanor, including the confidentiality of employee and client data, client boundaries, and other sensitive information.

- Provide evidence-based treatment that promotes client dignity, independence, individuality, strength, privacy and choice, recognizing the unique needs of the client population served by the agency.
- Provide the behavioral health services that the agency is authorized to provide and that the staff member is qualified to provide.
- Recognize obvious symptoms of a mental disorder, personality disorder, or substance abuse.
- Identify types of medications commonly prescribed for mental disorders, personality disorders and substance abuse, and the common side effects and adverse reactions to medications.
- Recognize, prevent and respond to a situation in which the client may be a danger to himself or others, behaves in an aggressive or destructive manner, is experiencing a crisis situation or medical emergency.
- Recognize and respond to fire, disaster, hazard, and medical emergency
- Remains flexible to all needs of SRC, including facilitation of group, individual/family meetings, assessments, and all other functions directed at all SRC facilities; and
- Has completed all of the requirements above and is documented in application in training including the ability to pass background checks required by Scottsdale Recovery Center.
