Position – Case Navigator
Job Summary:
The Case Navigator provides individualized, trauma-informed support to unhoused and precariously housed women and femme-identifying guests at Sister José Women’s Center. Working within SJWC’s low-barrier model, the Case Navigator helps guests identify goals, address immediate barriers, access community resources, and move toward housing stability, health, safety, and greater self-sufficiency. This role builds trusting relationships with guests through consistent engagement, compassionate communication, and practical support. The Case Navigator works one-on-one with guests to co-create housing and health stability plans, coordinate services, assist with documentation and benefits, and connect guests to housing, healthcare, behavioral health, employment, legal, and other essential resources. Reports to the Director of Program Services.
Sister José Women’s Center provides for the daily survival needs of unhoused women and supports their efforts to move forward in their lives through services, compassionate support, and respect for the dignity of each individual. The ideal candidate is flexible, innovative, consistent, has a deep understanding of the day-to-day needs of the guests, and a desire to compassionately serve the unhoused.
Duties/Responsibilities:
May include but are not limited to the following:
- Build trusting, respectful relationships with guests using trauma-informed, culturally responsive, strengths-based, and low-barrier practices.
- Meet regularly with assigned guests to assess needs, identify barriers, set goals, and monitor progress toward housing, health, employment, and stability outcomes.
- Assist guests with obtaining vital documents, including identification, birth certificates, Social Security cards, and other records needed for housing, benefits, employment, or healthcare access.
- Coordinate referrals and warm handoffs to community providers, including medical, behavioral health, employment, legal, housing, and benefits partners.
- Support guests in navigating systems such as AHCCCS, Social Security, Adult Protective Services, housing programs, healthcare providers, behavioral health services, legal services, and vocational resources.
- Provide supportive coaching in areas such as budgeting, financial literacy, resumes, cover letters, job interview preparation, professional behavior, and tenancy readiness.
- Help guests access approved financial assistance when available, including transportation support, housing-related costs, work clothing, medical needs, pet expenses, legal documents, and other client support needs.
- Participate in case staffing, team meetings, trainings, community coordination meetings, and internal planning efforts as assigned.
- Maintain accurate, timely, and confidential documentation of guest interactions, goals, referrals, services provided, progress, and outcomes.
- Respond to guest needs and conflicts with calm, respectful crisis de-escalation and safety-focused communication.
- Uphold SJWC’s mission, values, policies, confidentiality standards, and commitment to dignity, safety, inclusion, and nonjudgmental service.
- Perform other duties as assigned to support the needs of guests, the Case Navigation program, and the organization.
Qualifications and Abilities:
- Communicate with guests with a variety of behavior health concerns including serious mental illness and substance abuse, in a positive, caring, confidential, and professional manner that respects and understands the importance of personal boundaries.
- Build community and trust with vulnerable individuals to maintain a safe environment in which women can recuperate and move forward to a more sustainable life.
- Compassionately enforce Center policies and guidelines for guests, staff, and volunteers.
- Ability to prioritize tasks and multi-task.
- Strong communication, administrative, and interpersonal skills – written and oral.
- Proficient in Word, Outlook, Excel, and Teams
- De-escalation skills, crisis management, and trauma-informed care practices.
- Work cooperatively in a team environment.
- Able to make hard decisions, uphold personal and professional boundaries, and work with challenging behaviors.
Requirements:
- Bachelor’s degree in Human Services, Social Work, Social Services, or another related field or a High School diploma or General Education Equivalent (GED) and 4-8 years as a Case Worker or Case Manager.
- Must pass a pre-employment drug screening for illicit drugs and marijuana (THC) as a condition of employment.
- Must be able to pass a background check to work with vulnerable adults.
- Bilingual preferred.

Pay: $41,000.00 - $48,000.00 per year
