Full Job Description

Hospice Nurses –We are looking for Amazing RNs with at least 6 months - 1 year of in-home hospice experience, who can demonstrate compassion, and lead with a hospice heart. Due to growth, we are currently recruiting for a WEEKEND NURSE to join our team to do admission and triage visits.

Our hospice agency was founded by an RN owner right here in Arizona almost 30 years ago! We have grown to a regional company with a footprint across Arizona, but we operate with a true home-town feel. Each local branch is empowered to focus on the needs of the team and community they serve.

About the job:

Ideal candidates will be "Clinical Rockstars" who can balance Medicare compliance with the deep emotional intelligence required for hospice care. A call to hospice is a call for help; therefore, success is measured by both efficient response, compassion, and the clinical accuracy of the initial election.

Key responsibilities:

Conduct comprehensive evaluations of the patient's medical condition and collaborate with physician(s) to determine Eligibility. These evaluations can occur at a patient's residence or at a facility.
With a compassionate and empathetic approach, educate patients and families about the hospice philosophy and communicating how their Goals of Care align with hospice services.
Explaining to patients and families about what to expect from hospice care, the benefits it offers, and how it can enhance the patient's quality of life.
Setting up initial care needs for patients, including arranging necessary medications and medical equipment.
Coordinating with the interdisciplinary team to develop an initial plan of care tailored to the patient's needs.
Completing compliant and accurate documentation timely.
Serve as a brand ambassador in the community when meeting with families and patients, reflective of a high degree of integrity and customer service.
Perform triage or scheduled nursing visits (ex: service intensity care) or assist other teammates as time allows.

Qualifications:

RN License
As an RN, hospice patient care experience (Admissions, Triage, Case Manager)
Solid understanding of Medicare Hospice eligibility criteria, LCDs, and documentation requirements
Flexible and adaptable
Attention to detail
Solid assessment skills
Excellent verbal and written communicator
EMR experience - If you know Homecare Homebase (HCHB) that's a plus!
Can-do attitude and team player!
