Job Description

Position Title: Part Time Care Coordinator

Description: A care coordinator works under the direct supervision of the treatment program supervisor. The care coordinator’s goal is to provide support to the foster families in the program and ensure that services are being provided to the children placed in those foster homes. The Tulsa Care Coordinator is required to complete monthly home visits with our agency foster parents. This position is partial remote (working from home) and partially in the field (visiting foster parents). A company vehicle and gas card is provided.

Care Coordinating:

· Monthly staffing with supervisor regarding foster homes.

· Complete placement of children to include necessary documentation.

· Monthly in-home face-to-face contact with children and foster families - documentation of visits is required.

· Quarterly contact with homes with no children in placement.

· Weekly phone or text contact - documentation is required.

· Various case management duties: Example, contacting DHS worker, advocating for foster parents and children, school advocacy and others.

· Completing re-assessments as to OKDHS standards.

· Supervise visitations.

· Support foster home parent(s) with attaining documents needed for foster children.

· Assist/prepare for audits by DHS Child Welfare and Licensing.

· Coordinate, schedule, and take notes for monthly/quarterly treatment team meetings.

· Send treatment team meeting notes to foster child’s worker and supervisor.

Qualifications:

· Bachelor’s degree in Social Work or related field.

· Knowledge of community resources and counseling/social work practices with high-risk juvenile populations

· Experience working with persons in crisis.

· Good documentation skills.

· Excellent written and verbal communication skills.

· Ability to establish trust and rapport.

· Ability to motivate other towards achieving goals.

· Ability to work independently with a strong sense of focus, task-orientated, non-judgmental, open personal qualities and a clear sense of boundaries.

· A strong sense of respect for confidentiality involving both clients and fellow employees.

· Ability to work in a variety of settings with culturally diverse families and communities, as well as the ability to be culturally sensitive and appropriate.

· Ability to legally operate a motor vehicle and provide own transportation.

· Care Coordinator shall not exceed a 5 home caseload, without salary adjustment. This salary is based on a 5 home caseload.
