Full Job Description

Patient Health Navigator

Job Type- In Person

Salary: $22.00 per hour or Based on Experience

Position Summary:

Join the excellent staff of AllCare IPA, as a Patient Health Navigator located in Modesto, California.

AllCare is an Independent Physician Association, formed in 1989, and represented by over 1,000 private practice physicians in Stanislaus, San Joaquin and Merced Counties. AllCare's focus is on prevention of illness and promotion of a healthy lifestyle.

Job Responsibilities:

Serves as a liaison between internal and external customers, including patients, other AllCare staff and other service providers, for the purpose of improving patient quality of care.

Focuses on care gap closure for STAR and AMP measures including medication adherence, preventative screening, and care for diabetic patients.

Completes all required documentation accurately, in a timely manner, and thoroughly in accordance with Company standards.

Works in collaboration with all members of the program on retention, appointment completion, and identifying barriers to care (e.g., quality improvement projects, Preventative Screening Appointments, Medication Reminders, etc.).

Engages in patient and provider office interactions via telephone, text, and email to identify and assist with any barriers to care, facilitate referrals, and provider resources, depending on patient needs.

Performs general clerical tasks, as assigned, which may include filing, processing paperwork, data entry, scanning/photocopying, agenda creation and minutes, encounter reports, scheduling, and similar tasks.

Uses discretion during all interactions to answer both routine and complicated questions, forwards issues to appropriate person. Handles confidential and routine information.

Performs other duties as assigned.

Education/Skills:

Bilingual applicants highly preferred.

High School diploma (or GED equivalent) required.

2 years of experience with health education or working in the medical/health related field preferred.

Or equivalent combination of education and experience that provides the skills, knowledge, and ability to perform the essential job duties, and which meets any required state or federal certification requirements

Great customer service skills

Knowledge of Excel and Word

General medical knowledge and understanding of medical terminology
