Social Worker – PACE Program
Position Overview
We are seeking a compassionate and experienced Social Worker to support a PACE (Program of All-Inclusive Care for the Elderly) program in a full-time, onsite capacity. This role focuses on providing psychosocial assessments, care coordination, counseling, and support services for elderly participants and their families within an interdisciplinary healthcare environment.

This is a temporary assignment with strong potential for long-term opportunities based on performance and organizational needs.

Work Schedule
Full-time, onsite
Monday through Friday
8:00 AM – 4:30 PM

Key Responsibilities

Conduct initial, 6-month, and annual participant assessments
Collaborate with the interdisciplinary care team to develop and update individualized care plans
Provide emotional support, counseling, and crisis intervention to participants and families
Assess psychosocial needs including anxiety, depression, cognitive concerns, abuse risk, and safety planning
Coordinate care services including transportation, community resources, and family communication
Support discharge planning and transitions of care
Facilitate support groups and participant counseling activities
Assist with advance directives, placement planning, and financial/community resource coordination
Maintain accurate documentation and medical records
Participate in staff meetings, case conferences, and quality improvement initiatives
Conduct home visits as needed

Qualifications

Required

Master’s Degree in Social Work (MSW)

Minimum 2 years of experience in a healthcare-related setting

At least 1 year of recent experience working with the elderly population
CPR and First Aid certification (or ability to obtain within 6 months of hire)

Preferred

Case management experience with the elderly population

Bilingual abilities in Spanish, Vietnamese, Korean, Chinese, Arabic, Farsi, or Russian
