Overview

HealthCare Coordinator (Part-Time) Position Summary

We are seeking a detail-oriented and proactive professional to serve in a hybrid dual-role capacity as a Healthcare Coordinator. This part-time position combines provider network management responsibilities with patient outreach and education to support successful Transitional Care Management (TCM) services.

The ideal candidate will possess strong communication, negotiation, organizational, and relationship-building skills, with experience in healthcare administration, provider credentialing, payor contracting, and patient engagement.

Key Responsibilities Commercial Payor Contractor (Approximately 2 Hours Per Day)

Manage and complete commercial payor contracting activities to ensure all providers maintain in-network status with applicable insurance carriers.
Negotiate commercial payor contracts and reimbursement terms on behalf of the organization.
Maintain and update provider enrollment records and contract documentation.
Complete CAQH profile updates, attestations, and ongoing maintenance for all providers.
Monitor contract renewals, expirations, and payor requirements to ensure compliance.
Serve as a liaison between IPA leadership and insurance payors regarding network participation and credentialing matters.

Transitional Care (Approximately 2–3 Hours Per Day)

Travel to local healthcare facilities to educate eligible patients about the Transitional Care Management (TCM) program.
Explain program benefits, services, and patient participation requirements.
Build relationships with facility staff to support TCM Program
Assist with identifying eligible patients for enrollment in TCM services.
Coordinate communication between patients, providers, and care management teams as needed.
Maintain accurate documentation of patient interactions and outreach activities.
Support initiatives designed to improve patient outcomes and reduce hospital readmissions.

Qualifications Required

Strong negotiation and communication skills.
Ability to work independently and manage multiple responsibilities effectively.
Reliable transportation and willingness to travel locally to healthcare facility.
Reliable Internet.

Preferred

Experience with provider enrollment and credentialing.
Experience working directly with patients in a healthcare setting.
Experience in healthcare administration, provider credentialing, payor contracting, or related healthcare operations.
Knowledge of commercial insurance payor networks and contracting processes.
Experience managing and maintaining CAQH provider profiles.

Schedule

Part-time position requiring approximately 4–5 hours per day:
