We are seeking a dedicated and energetic Weekend Care Coordinator to join our dynamic team. In this vital role, you will serve as a key point of contact for patients and their families during weekend hours, ensuring seamless coordination of care across various medical disciplines. Coordinates care through assessments, training, caregiver support, and communication with clients and families., all while maintaining a compassionate and patient-centered environment. This position offers an exciting opportunity to make a meaningful difference in the lives of individuals with diverse healthcare needs

Responsibilities

Conduct client and caregiver assessments
Provide on-site training and introductions
Serve as caregiver support resource
Participate in on-call rotation
Assist with orientation
Maintain documentation

Qualifications

Caregiving or healthcare coordination experience
Strong communication skills
Ability to travel locally
