Coordinates care through assessments, training, caregiver support, and communication with clients and families., all while maintaining a compassionate and patient-centered environment. This position offers an exciting opportunity to make a meaningful difference in the lives of individuals with diverse healthcare needs

Responsibilities

Conduct client and caregiver assessments
Provide on-site training and introductions
Serve as caregiver support resource
Participate in on-call rotation
Assist with orientation
Maintain documentation

Qualifications

Caregiving or healthcare coordination experience
Strong communication skills
Ability to travel locally
